Assessment of antimicrobial resistance laboratory-based surveillance capacity of hospitals in Zambia: findings and implications for system strengthening

背景

十分に確立された抗菌薬耐性の検査に基づくサーベイランスは、世界の伝染性疾患の負荷において大きな割合を占めているザンビアのような国において、極めて重要性が高い。本研究では、ザンビアにある選択した病院の検査室について、抗菌薬耐性サーベイランスを実施する能力を評価した。

方法

本探索的横断研究は、2023 年 8 月から 2023 年 12 月にザンビアで意図的に選択した病院 8 施設で実施した。データの収集は、自己判定式Laboratory Assessment of Antibiotic Resistance Testing Capacity（LAARC）ツールを用いて行った。

結果

評価対象とした病院において、抗菌薬耐性サーベイランスを実施する十分な能力を有する病院はなく、実施能力には中程度（63％［8 施設中 5 施設］）から低度（38％［8 施設中 3 施設］）までばらつきがあった。抗菌薬耐性の検査に基づくサーベイランスの実施における障壁は、電子検査情報システムの欠如（63％［8 施設中 5 施設］）および地域で作成されたアンチバイオグラムの欠如（75％［8 施設中 6 施設］）などであった。抗菌薬感受性試験に対する品質管理、病原体の同定、および培地の調製は、それぞれ 14％、20％および 44％と、全病院において全般的スコアが最低であった。全般的スコアが最高であったのは、検体の処理（79％）、データの管理（78％）、検体の収集／輸送／管理（71％）、および安全（70％）についてであった。ほとんどの病院は、標準作業手順を使用していたが、検体のための標準作業手順は存在していなかった。

結論

抗菌薬耐性サーベイランスを十分に実施する能力を有する検査室が存在しないことは、抗菌薬耐性と闘う取り組みを妨げており、感染性疾患の治療転帰をさらに複雑にしている。検査に基づくサーベイランスシステムを確立し強化することは、抗菌薬耐性の負担を定量化する上で、また地域におけるアンチバイオグラムおよび治療ガイドラインの開発を支援する上で、極めて重要である。
