背景
医療環境におけるスマートフォンは、病原性細菌の定着により感染症のリスクをもたらす。
目的
本パイロット研究では、消毒法効果持続期間について、70%イソプロピルアルコールワイプと短波長紫外線(UV-C)照射ボックスに焦点を当て、消毒後 3 時間における総細菌数減少に関する予備データを得ることを目的とした。
方法
2 つの介入群(ワイプおよび UVC ボックス)から成る単一施設ランダム化試験をデザインした。参加者として Fondazione Policlinico Universitario ‘A. Gemelli’ IRCCS Hospital の 3 病棟で医療従事者を登録し、病棟により層別化して、交絡因子を制御するために各病棟内でブロックランダム化を行った。
結果
医療従事者 71 名(そのうち大半は看護師[62%])を、本研究に組み入れた。最初の細菌数減少は、両消毒法ともにかなり大きかったが、3 時間後には両消毒法ともに細菌数の増加がみられ、ワイプの方が残存効果が高い傾向が示された(P = 0.056)。アルコール浸漬ワイプと UVC ボックスの消毒後 3 時間における残存効果を比較するための適切な対象被験者数(有意水準 0.05 で 89%の検出力)として、各群で医療従事者 503 名が必要であった。
結論
本研究では、病院におけるスマートフォン消毒に関する、また医療従事者および患者に対する教育イニシアティブに関するガイドラインの必要性が強く示された。最適な消毒間隔を明らかにするため、また病原体伝播のリスクを評価するために、十分な対象被験者数を組み入れたさらなる研究が必要である。
監訳者コメント:
医療現場におけるスマートフォンは広く普及しており、細菌伝播の原因となりうる。一般的に普及しているイソプロピルアルコールワイプと紫外線(UV-C)照射で、 3時間後の細菌数の減少を比較しているが、どちらも効果があり、どちらかというと特殊な装置を必要としないイソプロピルアルコールワイプの方が残留効果が高い可能性がありそうで、実用的な結果になっている。あまり神経質になってもしょうがないが、日常生活でのスマートフォンにも活用できそうである。
