背景

医療従事者の麻疹に対する免疫は特に懸念される。彼らは職業による曝露のために、一般集団よりも麻疹に感染する可能性が高い。彼らの職業による曝露は、院内感染のアウトブレイクを引き起こす可能性がある。

目的

スペインの 5 つの大学病院で、医療従事者の麻疹に対する免疫状態を評価すること。

患者と方法

5 つの大学病院の医療従事者を対象に、麻疹に対する化学発光間接免疫測定法による血清検査(IgG)を、前向きに連続で実施した。医療従事者全員を次の 4 つの疫学的な群に分類した。ワクチン接種を受けていた人、麻疹の既往歴があった人、麻疹の既往歴もワクチンの接種歴もなかった被験者、麻疹があったかどうか、ワクチン接種を受けたかどうか分からなかった人。また、次の 5 つの仕事のカテゴリーにも分類した。医師、看護師、看護助手、その他の臨床に従事する人、臨床に従事しない人。麻疹への免疫と独立に関連する因子を特定するために、ロジスティック回帰モデルを組み立てた。

結果

研究対象群は 2,157 名の医療従事者であった。89%には麻疹に対する防御抗体があった。免疫のなかった医療従事者 238 名のうち 199 名(83.6%)がワクチン接種を受けていたのに対して、免疫のあった 1,919 名では、1,084 名(56.5%)であった(P < 0.0001)。麻疹に対する防御抗体があることを有意に予測したパラメーターは、高齢(P < 0.0001)。疫学的状態(P = 0.0002、主に過去の麻疹による疾患)、仕事のカテゴリー(P = 0.02、特に看護師)であった。

結論

この研究は、以前にワクチン接種を受けていた人を含む医療従事者が、現時点で麻疹のリスクに曝されていることを明らかにし、感染の自然歴のある人の方がより防御されていることを示唆している。したがって、麻疹に対する免疫について知っていることと免疫を維持することは、医療従事者における感染制御の不可欠な部分である。

監訳者コメント:

医療従事者における麻疹 IgG 抗体を測定したところ、過去の感染歴のある医療従事者では抗体陽性を維持していた。また看護師は日頃から患者に接することが多いため、自然曝露によるブースターがかかっていた可能性がある。一律に MMR ワクチン接種するよりも、抗体検査後に十分な抗体が認められない医療従事者に対してのみワクチン接種をするのが費用対効果の面で良い。なお、日本環境感染学会のワクチンガイドライン第 3 版では 2 回の接種歴があれば、新たな追加接種を求めていない。
