Effectiveness of hand hygiene campaigns and interventions across the League of Arab States: a region-wide scoping review

手指衛生は感染症の蔓延を抑えるための標準的な公衆衛生活動であるが、依然としてルーチンでの全体的な衛生行動にはなっていない。本総説の目的は、アラブ連盟諸国で実施されたさまざまな手指衛生介入の効果を調査すること、既存文献のギャップを特定すること、ならびに今後の研究および介入の開発の領域を提示することである。2023 年 10 月 31 日同日までに発表された関連文献において、16 のデータベースを用いてスコーピングレビューを実施した。40 報の研究が選択基準を満たし、このうち 34 報が病院ベースの研究、6 報が地域ベースの研究であった。レビューした研究のうち 24 報では、介入の再現を可能にする十分な詳細が示された。18 報の研究は、世界保健機関 5 つのタイミングに一部変更を加えたものを、介入の内容または評価に使用していた。半数を超える研究（25 報）が、アウトカムとして医療従事者または学生の手指衛生行動を報告し、15 報では患者中心の何らかのアウトカムも含まれた。6 報は評価デザインまたは介入内容における理論またはフレームワークの使用について明記し、4 報は地方自治体のガイドラインまたは推奨事項の使用について記述していた。今後の研究では、地域ベースの研究を重視し、そして文化差を介入デザインに統合することで、文献のギャップの橋渡しをすることに焦点を置くべきである。さらに、手指衛生の研究への理論的フレームワークの適用は、理解ならびに効果を高め、アラブ連盟諸国の多様な環境において衛生習慣の改善が確実に持続できるであろう。

文化的な背景が科学的な医療水準を満たせない大きな障壁となっている地域が、未だに存在することがこの論文から知らされる。確かに特定の施設内では大きな変革がもたらされ、感染対策も推進されるであろうが地域や国全体の取り組みには時間がかかるかもしれない。
