消化管手術における切開手術部位感染症の予防を図る抗菌コーティング縫合糸を用いた筋膜閉鎖法の有効性：システマティックレビューおよびメタアナリシス★

本研究の目的は、日本外科感染症学会（JSSI）の手術部位感染症（SSI）予防ガイドラインの改訂の一環として、特に消化管手術における SSI の予防を図るために抗菌縫合糸を用いた筋膜閉鎖法の有効性に関して、システマティックレビューおよびメタアナリシスを実施することである。2023 年 5 月に CENTRAL、PubMed、ICHUSHI-Web で検索し、消化管手術における筋膜閉鎖法について抗菌コーティング縫合糸と非コーティング縫合糸を比較する無作為化対照試験（RCT）を含めた（PROSPERO No. CRD42023430377）。3 名の著者が独立して RCT を選択した。抽出データのバイアスリスクと GRADE 基準を評価した。主要評価項目は切開 SSI とし、副次評価項目は腹壁裂開および術後入院期間とした。本研究は、JSSI により一部支援を受けた。RCT 計 10 件、患者計 5,396 例を対象とした。抗菌コーティング縫合糸の使用は、非抗菌コーティング縫合糸と比較して、切開 SSI のリスクを有意に低下させた（リスク比 0.79、95％信頼区間 0.64 ～ 0.98）。サブグループ解析では、開腹手術およびモノフィラメント縫合糸使用の場合、抗菌コーティング縫合糸は SSI リスクを低下させた。抗菌コーティング縫合糸は、非コーティング縫合糸と比較して、腹壁裂開発生率の低下および入院期間の短縮をもたらさなかった。GRADE 基準により、エビデンスの確実性は、バイアスリスクのために中等度と評価された。結論として、消化管手術における筋膜閉鎖法での抗菌コーティング縫合糸の使用は、非抗菌コーティング縫合糸よりも SSI リスクの有意な低下と関連する。

この研究によりシステマティックレビューおよびメタアナリシスの結果、消化管手術における筋膜閉鎖法での抗菌コーティング縫合糸の使用は非抗菌コーティング縫合糸よりも SSI リスクの有意な低下が得られたという。大変興味深い。
