目的

全員を対象としたメチシリン耐性黄色ブドウ球菌(MRSA)のスクリーニングを行うには、追加の費用がかかり、高リスクの集団を標的としたスクリーニングについては、コンセンサスが存在しない。この研究の目的は、高齢者介護部門における MRSA の存在率を明らかにし、入院時における MRSA の定着に関連する因子を特定することであった。

方法

ベルギーの 6 つの病院の高齢者介護部門におけるこの後向き症例対照研究(1:1)は、2021 年 1 月 1 日から 2022 年 12 月 31 日の期間が対象であった。症例は、高齢者介護部門に入院してから 48 時間以内の MRSA のスクリーニング結果が陽性であった患者とし、対照は、スクリーニング結果が陰性であった患者とした。

結果

合計で、556 例の患者がこの研究に組み入れられた(各群 278 例)。入院 100 件当たりの存在率は、全検体で、2.3(95%信頼区間[CI]2.2 ~ 2.6)であった。入院時において MRSA の保菌者であったことと関連していた有意な多変量因子は、MRSA の既往、介護施設出身、慢性の皮膚病変であった。これら 3 つの因子を適用すると、受信者動作特性(ROC)曲線下面積は 0.73(95%CI 0.71 ~ 0.77)となり、スクリーニングを症例の 55.4%(95%CI 51.2 ~ 59.6%)のみを対象として実施することが可能となるだろう。

結論

高齢者介護部門でこれらの因子をスクリーニングの基準として使うことで、MRSA についてスクリーニングする患者の数をかなり減少させ、その一方で、満足のいく感度と特異度を維持することが可能である。

監訳者コメント

高齢者の入院患者において MRSA のユニバーサル・スクリーニングは困難であるが、MRSA スクリーニングを行う条件として MRSA の既往、介護施設、慢性皮膚病変などの因子を有する患者が挙げられる。
