Profiles of central nervous system surgical site infections in endoscopic transnasal surgery exposing the intradural space

目的

経鼻内視鏡手術は有効性および最小限の侵襲性にかかわらず、準清潔の状態であるため、特に硬膜露出を伴う場合、中枢神経系の手術部位感染症（SSI）を起こしやすい。しかし、経鼻内視鏡手術関連の中枢神経系 SSI のプロファイルは十分に示されていない。

方法

2017 年 5 月から 2023 年 3 月に当施設で実施された経鼻内視鏡手術の症例を後向きに分析した。中枢神経系 SSI 発生率を算出し、そのリスク因子を調査した。

結果

中枢神経系 SSI 発生率は、全コホートでは 2.3％（305 件中 7 件）、硬膜露出を伴う経鼻内視鏡手術では 5.0％（140 件中 7 件）であった。中枢神経系 SSI はすべて髄膜炎で、硬膜露出を伴う経鼻内視鏡手術後に発生した。経鼻内視鏡手術中における Esposito グレード 1、2、3 の脳脊髄液漏出の発生率は、それぞれ 0％、5.6％、5.8％であった。術前および術中の因子のうち、体格指数（単位オッズ比［OR］0.62、95％信頼区間［CI］0.44 ～ 0.89、P ＜ 0.01）、血清アルブミン（単位 OR 0.03、95％CI 0.0007 ～ 0.92、P ＝ 0.02）、American Society of Anesthesiologists の全身状態スコア（単位 OR 20.7、95％CI 1.65 ～ 259、P ＜ 0.01）は、中枢神経系 SSI と有意に関連した。さらに、術後の脳脊髄液漏出も中枢神経系 SSI と有意に関連した（OR 18.4、95％CI 3.55 ～ 95.0、P ＜ 0.01）。

結論

経鼻内視鏡手術関連の中枢神経系 SSI の発生率は、許容できる程度に低い。硬膜露出は中枢神経系 SSI を引き起こす前提条件であった。低栄養状態および併存疾患の不良な状態は、経鼻内視鏡手術における中枢神経系 SSI の重大なリスクとして認識すべきである。
