Establishing semi-automated infection surveillance in obstetrics and gynaecology

背景

サーベイランスは、手術部位感染症（SSI）などの院内感染症を低減するための方法として認められている。電子カルテにより、自動サーベイランスを実施する機会が生まれている。手術において異なる種類の術式および指標に関するアプローチは既に存在する一方、産婦人科においてはそうしたアプローチが非常に少ない。

目的

産婦人科における半自動サーベイランスの感度および作業負担軽減効果について分析すること。

方法

ドイツにある 1,438 床の 3 次病院における本単一施設後向き研究で、指標として「抗菌薬処方（antibiotic prescription）」「微生物学的データ（microbiological data）」および「管理データ（administrative data）」（診断コード、再入院、退院後ケア）を用いて、半自動 SSI サーベイランスを、手動による全患者ファイルの解析およびカテゴリー化と比較した。2018 年に組み入れ基準を満たした乳房手術、ならびに 2013 年 5 月から 2019 年 12 月に組み入れ基準を満たした帝王切開を組み入れた。指標について、感度、1 症例を同定するために解析対象として必要とされた手技数、ならびに SSI 検出における作業負担軽減作用となった可能性を、対照グループと比較して解析した。

結果

参照基準とした手動サーベイランスでは、乳癌手術 416 件において SSI 9 件（2.2％）が認められた。「抗菌薬処方」「診断コード」「採取された微生物学的サンプル」の指標、および「診断目標」または「微生物学的サンプル」の組み合わせの感度は、それぞれ 100％、88.9％、66.7％および 100％であった。参照基準では、帝王切開 3,438 件においてSSI 54 件（1.6％）が認められた。「微生物学的サンプルの収集」「診断コード」「再入院／退院後ケア」の指標、および全指標の組み合わせの感度は、それぞれ 38.9％、27.8％、85.2％および 94.4％であった。

結論

半自動サーベイランスシステムは、手術の種類、現場の状況および徹底したデジタル化に依存して高い感度を維持しながら、作業負担を削減する可能性がある。
