汚染水を含んだ病院機器が原因のアクロモバクター（Achromobacter）属菌アウトブレイク：感染症のリスク因子★

背景
アクロモバクター（Achromobacter）属菌が原因の尿路感染症の院内アウトブレイクは、ここ数十年間ではまれであった。
目的
Achromobacter 属菌アウトブレイクの発生源を同定すること、多面的感染制御策を実施すること、そして最終的にアウトブレイクを終息させること。この目的のため、疫学的アウトブレイク調査およびリスク因子解析を実施した。
方法
Achromobacter 属菌が、当院の泌尿器科病棟における患者 22 例、および手術室から採取され検体の環境培養 6 件で検出された。分離株を抗菌薬感受試験に供した。施設内での疫学調査、患者カルテの評価、および環境サンプル採取を行って、このアウトブレイクの発生源の特定を試み、感染制御介入を実施した。症例対照研究を実施して、可能性のあるリスク因子の解析を行った。
結果
環境サンプル採取から、患者 22 例における感染症の発生源は汚染水を含んだ ISA-IIIA 型医療用加圧器であることが示された。症例対照研究の解析から、感染症のリスク因子は、腎結石・尿道結石の診断、手術、ダブルジェイステントの留置、および過去 3 か月間における入院歴であることが示された。
結論
結論として、今回のアウトブレイクは、体内砕石術およびダブルジェイステントの留置を受けた患者で発生しており、ISA-IIIA 型医療用加圧器の汚染されたセンサーおよび接続チューブとの接触による伝播が原因であった。
