地域の外科施設のプロトコルへの皮膚処置ガイドラインの取り込み：どのような障壁に直面するか？フランスにおける多施設研究★

背景
手術部位感染症（SSI）は、術後もっとも頻度が高い重症の有害事象である。予防策において、ルーチン業務の術前皮膚処置の実施にばらつきがあることが知られている。フランスの地域における手術プロトコルに、ガイドラインを実際に取り込むことが必要不可欠であろう。
目的
術前皮膚処置の推奨が、地域のプロトコルに取り込まれているかどうかを評価し、取り込まれていない理由を特定すること。
方法
フランス全国サーベイランスおよび SSI 予防ネットワーク Spicmi に参加している施設のすべての感染制御チームにオンライン調査を提示した。参照の推奨事項は、French Society for Hospital Hygiene ガイドラインに準拠した。
結果
全体で 485 の医療施設が質問票に回答した。施設のプロトコルへの推奨の取り込みは、推奨事項によって 30％から 98％と異なった。もっとも多く取り込まれた方策は、アルコール製品による消毒、全身除毛の中止であった。取り込みの頻度がもっとも少なかったのは、術前シャワーでの普通石けんの使用、手術室での任意の皮膚洗浄であった。報告された障壁は、術前皮膚処置に特有のもの（例：「SSI の増加についての懸念」「推奨事項についての疑念」「惰性」）または非特有のもの（例：「プロトコルがまだ更新されていないため」）のいずれかであった。
結論
地域のほとんどの施設のプロトコルに、いくつかの主要な予防策が取り込まれているが、以前の推奨事項に基づく、エビデンスが欠如した方策もまだ多く含まれることが示唆される。SSI 発生率の推移に関する情報のやりとり、学会によるガイドラインの普及、司法専門家との交流により、推奨事項を適用する条件が明確になるであろう。
監訳者コメント：
ガイドラインに準拠した術前皮膚処置の実施は、施設間でばらつきがある。さらなるエビデンスの蓄積、遵守状況の改善が必要である。
