背景
アゾール耐性アスペルギルス・フミガーツス(Aspergillus fumigatus)は、世界的な公衆衛生上の懸念として報告されているが、様々な国で、予想外に観察されてきた。
目的
様々な病院の環境検体で、アゾール耐性 A. fumigatus を同定し、CYP51A の変異に関係したアゾール耐性を検出すること。
方法
このイランからの多施設共同研究では、イランの様々な病院の電子装置と機器の表面から、綿棒を使って、検体を採取した。全検体を、アゾールを含んだ寒天平板を使って培養した。回収された Aspergillus の分離株は、β-チューブリン遺伝子を部分的に DNA シークエンシングすることによって、種のレベルで同定した。A. fumigatus 分離株のアゾール感受性試験は、米国臨床検査標準化協会(Clinical and Laboratory Standards Institute)の M38-A3 ガイドラインを使って実施した。耐性に関係した変異を検出するために、CYP51A 遺伝子のシークエンシングも実施した。
結果
収集した 693 個の検体のうち、89 個(12.8%)で、アゾールを含んだ寒天平板から Aspergillus 属が回収された。A. fumigatus(41.6%)が最もよくみられ、続いて、A. tubingensis(23.6%)、A. niger(15.6%)であった。A. fumigatus の 37 の単離株のうち 19 株(51.3%)で、ボリコナゾール、イトラコナゾール、ポサコナゾールの 3 つのアゾールの少なくとも 1 つに対する最小発育阻止濃度(MIC)値が高くなっていた。CYP51A の多型が 19 株すべてで検出され、そのうち 52.6%が TR34/L98H 変異であった。検出されたその他の変異は、G432C、G448S、G54E/G138C、F46Y、Y121F/M220I/D255Eであった。T289F と G432C は、アゾール耐性 A. fumigatus で初めて報告された変異であった。
結論
病院の環境検体中にはかなりのレベルのアゾール耐性が存在した。これは、アスペルギルス症に罹りやすい患者にとって、深刻な警告である。我々の結果は、CYP51A 遺伝子の変異の異なるパターンも明らかにした。
監訳者コメント:
アスペルギルスは環境中に広く分布する糸状真菌であり、深在性真菌症、なかでも侵襲性肺アスペルギルス症は免疫不全患者の生命予後に影響するため、早期診断と早期治療が必要である。近年、アゾール耐性アスペルギルスが世界的な問題となり、世界保健機関においても対策の必要な最重要病原体に位置づけられている。本論文では、電子機器類の表面拭いの培養において検出された A. fumigatus の半数がアゾール耐性であったことは衝撃的である。すでに日本においても耐性菌は報告されており、特に臓器移植や造血幹細胞移植を受けた患者の適切に維持管理された保護的環境の確保と早期診断のための真菌の同定および薬剤感受性の検査体制整備が吃緊の課題である。
