新生児集中治療室における擦式アルコール製剤の乾燥時間の遵守を改善するための手指衛生の視覚的フィードバックシステムの検証：Incubator Traffic Light システム★

背景
擦式アルコール製剤の推奨乾燥時間 30 秒の遵守は不十分なことが多い。新生児集中治療室（NICU）での手指衛生遵守を向上させるために、推奨乾燥時間の経過後に保育器の扉を開けるための「青信号」を表示する Incubator Traffic Light システムを設置した。
目的
この視覚的フィードバックシステムが、NICU 医療従事者における擦式アルコール製剤の推奨乾燥時間の遵守に及ぼす影響を評価すること。
方法
NICU の保育器に 10 の traffic light システムを設置し、そのうち 5 つは視覚的フィードバックを提供し、5 つは視覚的フィードバックを提供しない対照群とした。2 か月間にわたり、擦式アルコール製剤の適用時から保育器の扉を開けるまでの乾燥時間をシステムにより計測した。フィードバックの有無により、保育器に表示される乾燥時間を比較した。
結果
手指衛生イベントを記録した 6,422 件のうち 658 件がデータ分析に適用された。適切な乾燥時間の遵守は、視覚的フィードバックを設置した保育器では 75％（526 件中 397 件）に達したのに対し、この機能のない保育器では 36％（132 件中 48 件、P ＜ 0.0001）であった。
結論
NICU 環境において、Incubator Traffic Light システムは擦式アルコール製剤の推奨乾燥時間 30 秒の遵守を改善する。

Incubator Traffic Light は、アルコールによる手指消毒の推奨乾燥時間を守る上で、一助となるかもしれない。
