外科病棟の新築後 1 年間における患者と病院環境間のカルバペネム耐性グラム陰性菌の交差感染

背景
病院におけるカルバペネム耐性グラム陰性菌の伝播とアウトブレイクは、廃水環境の汚染と関連することが多い。ドイツの Heidelberg 大学病院に新設された建物の外科ステップダウン病床および集中治療室の入室から 1 年間、病院廃水環境の細菌叢を調査するために前向き観察研究を実施した。
方法
廃水システム（トイレ、シンク）の月 1 回のスクリーニングを、開設 1 か月前から 12 か月間（2020 年 10 月 1 日から 2021 年 10 月 30 日まで）実施した。また、患者におけるカルバペネム耐性グラム陰性菌の直腸スクリーニングを入院時および週 1 回実施した。細菌分離株を全ゲノムシークエンシングにより確認した。
結果
入院時に患者 1,978 例のうち 27 例（1.4％）で、カルバペネム耐性グラム陰性菌の保菌／感染症がみられた。試験期間に患者 24 例から分離されたカルバペネム耐性グラム陰性菌 29 株と、環境サンプルから分離された 52 株をシークエンシングに用いた。入室から 1 か月以内に、カルバペネム耐性グラム陰性菌の保菌／感染症の患者 7 例を特定したが、環境リザーバには認められなかった。下水道にカルバペネム耐性グラム陰性菌株が検出され始めたのは、最初の入室から 5 か月後であった。保菌歴のない患者 2 例で、下水道に定着する緑膿菌（Pseudomonas aeruginosa）株の保菌／感染症が認められた。カルバペネマーゼ遺伝子を保有するプラスミドの顕著な同一性は、下水道への長期間の定着が新たなカルバペネム耐性クローンの発生を促すことを示唆している。
結論
患者と病院環境間の交差感染は双方向的である。本研究では、病院の廃水環境の汚染がカルバペネム耐性グラム陰性菌の持続的な定着を引き起こし、院内獲得のリザーバとして機能する可能性があることを明らかにした。
監訳者コメント：
患者と病院環境のカルバペネム耐性菌の全ゲノム解析により、新病棟における患者の保菌／感染と下水回り（トイレ／シンク）の汚染との関連を示した。
