ICU におけるグラム陰性菌による感染症または保菌のリスク低減を図るシンクへの介入：文献のシステマティックレビュー

背景
集中治療室（ICU）において医療関連感染症（HAI）は重要な問題である。病院の水環境はグラム陰性菌（GNB）のリザーバとなる可能性があり、アウトブレイクおよび非アウトブレイクの状況下、汚染シンクは GNB 拡散の一因となることが示されている。本研究の目的は、ICU においてシンクへのどの介入が GNB 感染発生率および保菌率を低下させうるか検討することである。
方法
言語および公開日の制限を設けず、データベースの検索（MEDLINE via PubMed、EMBASE via Ovid、ClinicalTrials.gov）を行った。病室の水道設備への介入が報告されていること、また非アウトブレイク状況下で HAI 発生率または保菌率のデータが示されてことを条件に、あらゆるデザインの研究を対象とした。GNB および緑膿菌（Pseudomonas aeruginosa）の獲得（感染および／または保菌）率をアウトカムとして分析した。
結果
全体で 4,404 報の論文が特定された。11 報が最終分析の対象となった。分析に無作為化対照試験は含まれず、すべての研究で中等度から高度のバイアスリスクが示された。シンクの除去および蛇口フィルターの取り付けが、GNB 獲得に顕著な影響を及ぼしたが、研究で報告されたアウトカムおよびサンプルサイズにおいて異質性が高かった。
結論
非アウトブレイク状況下で、病室のシンクと医療関連の GNB 獲得との関連を検討した研究はわずかであった、研究デザインの異質性により、結果の一般化は不可能であった。ICU において病室からシンクを除去することで、特有の HAI 発生率を低減できるか否かをさらに検討するために、前向き試験が必要とされる。
監訳者コメント：
ICU におけるシンクへのHAI 発生率の影響は、きわめて重要である。
シンクまわりの衛生状況がいかに大きく影響するかということについては、よく理解する必要がある。
