背景
中心ライン関連血流感染症(CLABSI)は、相当な罹患率と病院費用を伴う重篤な医療関連感染症である。
目的
2013 年から 2018 年までの、ギリシャの 3 次病院における CLABSI の発生率、特定の感染制御策の実行、多剤耐性(MDR)菌血症の発生率の間の関連性を検討すること。
方法
毎月算出される次の指標に分析を適用した:CLABSI 発生率、手指衛生消毒薬の使用、MDR 菌を有する患者の隔離率、菌血症の発生率(グラム陰性カルバペネム耐性アシネトバクター・バウマニー[Acinetobacter baumanii]、カルバペネム耐性緑膿菌[Pseudomonas aeruginosa]、およびカルバペネム耐性肺炎桿菌[Klebsiella pneumoniae]の合計、グラム陽性メチシリン耐性黄色ブドウ球菌[Staphylococcus aureus]およびバンコマイシン耐性腸球菌[vancomycin-resistant enterococci])。
結果
カルバペネム耐性グラム陰性病原体由来の菌血症の総数は、病院の全診療科(発生率比[IRR]1.17、95%信頼区間[CI]1.05 ~ 1.31、P = 0.006)および成人集中治療室(ICU)(IRR 1.37、95%CI 1.07 ~ 1.75、P = 0.013)の CLABSI 発生率の上昇と有意に相関していた。成人 ICU では、耐性を有する各グラム陰性病原体の発生率の上昇は、いずれも CLABSI の発生率低下と有意に相関していた(カルバペネム耐性 A. baumanii では IRR 0.59、95%CI 0.39 ~ 0.90、P = 0.015、カルバペネム耐性肺炎桿菌では IRR 0.48、95%CI 0.25 ~ 0.94、P = 0.031、カルバペネム耐性緑膿菌では IRR 0.54、95%CI 0.33 ~ 0.89、P = 0.015)。手指消毒薬の使用は、病院の全診療科に対する本介入の適用の 1 ~ 3 か月前の CLABSI 発生率の低下(IRR 0.80、95%CI 0.69 ~ 0.93、P = 0.005)と相関しており、当月の擦式消毒薬の使用は、成人 ICU の CLABSI 発生率の低下(IRR 0.34、95%CI 0.11 ~ 1.03、P = 0.057)と有意に相関していた。MDR 病原体を有する患者の隔離は、CLABSI 発生率に関連していなかった。
結論
手指衛生は、試験実施病院における CLABSI の発生率の有意な低下に関連していた。時系列分析は、感染制御介入を評価する重要なツールの 1 つである。
監訳者コメント:
よくあるといえばよくある時系列分析を用いた医療関連感染症の発生頻度の経時的評価で、手指衛生遵守率との相関が認められた。しっかりと評価を行えば、単一施設の取り組みでも JHI に掲載される、という良い見本。
