術前抗菌薬投与により整形外科用インプラント除去後早期の手術部位感染症が減少する:傾向をマッチさせたコホート研究

背景
整形外科における骨折固定後のインプラント除去はよくある手術であるが、依然として手術部位感染症(SSI)の高い発生率を伴う。抗菌薬予防投与は推奨されていないが、一部で支持されている。
目的
整形外科用インプラント除去後早期の SSI の予防における抗菌薬予防投与の有効性を評価すること。
方法
単一施設後向きコホート研究を実施した。2016 年から 2021 年に整形外科用インプラント除去術を受けた患者を対象とした。1:1 傾向スコアマッチング関数を用いて、ベースライン時の特性および関連する SSI のリスク因子をマッチさせたコホートを設定した。傾向スコアマッチングの後、SSI(表在性または深在性)および SSI に対する修正手術の発生率を、オッズ比を用いてコホート間で比較し、術前の抗菌薬予防投与の効果を検討した。
結果
全体で、重複しない外科手術 965 件を組み入れた。このうち 69 件(7.15%)で SSI が発生し、69 件中 24 件(35.7%)で外科的修正を要した。214 件(22.18%)が術前抗菌薬予防投与下で行われた。傾向をマッチさせたコホートは、手術 396 件(抗菌薬予防投与あり/なし各 198 件)で構成された。SSI 発生率は、対照群で 11.11%、抗菌薬予防投与群で 3.03%であった(オッズ比 0.25、95%信頼区間 0.099 ~ 0.63、P = 0.011)。修正手術については差は認められなかった。
結論
術前の抗菌薬予防投与により、整形外科用インプラント除去時の SSI のリスクが大幅に低下し、副作用のリスクの上昇は伴わない。
監訳者コメント:
整形外科用インプラント除去時には術前の抗菌薬予防投与が行われているものと想像していたが、実際には投与されていないケースがあるのかもしれない。術前抗菌薬の有無によって SSI の発症率には大きな差があり、一度自施設の状況を確認しても良いかもしれない。
