英国ウェールズにおける 2020 年 2 月から 2022 年 3 月の入院 COVID-19 症例の感染源別およびパンデミック波別の死亡率

背景
入院患者は COVID-19 に対して脆弱であるが、病院感染例と市中感染例とを比較した相対重症度は不明である。本研究では、ウェールズにおける病院感染症例と市中感染症例の死亡率の差を調査した。
方法
2020 年 2 月から 2022 年 3月の重症急性呼吸器症候群コロナウイルス 2（SARS-CoV-2）ポリメラーゼ連鎖反応検査を、入院と関連付け、病院感染の可能性がある症例を特定した。SARS-CoV-2 陽性から 28 日以内の全死因死亡率を、感染源別に計算した。多変量ロジスティック回帰を用い、交絡因子の補正を行い、補正オッズ比（aOR）と 95％信頼区間（CI）を算出して、感染源別の死亡率を比較した。
結果
研究期間中の COVID-19 の病院感染症例は 25,263 例であり、5,490 例（22％）が死亡した。単変量解析では有意差が認められたが、交絡因子の補正後には、病院感染症例において、COVID-19 により入院した症例と比較して、死亡率上昇との関連は認められなかった（aOR 0.8、95％CI 0.7 ～ 0.8）。ワクチン接種（aOR 0.6、95％CI 0.5 ～ 0.7）および後期のパンデミック波での感染（aOR 0.5、95％CI 0.4 ～ 0.6）に死亡率低下との関連が認められ、高齢（85 歳以上 対 25 歳未満：aOR 76.4、95％CI 41.8 ～ 160.5）および男性であること（aOR 1.5、95％CI 1.4 ～ 1.6）に死亡率上昇との関連が認められた。
結論
入院した COVID-19 症例5例に1例が、感染から 1 か月以内に死亡した。院内発症例の死亡率は、COVID-19 により入院した患者と比べて高くはなく、これはおそらく、スクリーニングによって院内発症例は早期に発見されることを反映している。
