背景
低所得国では、手術部位感染(SSI)は極めて頻度が高い病院感染の 1 つである。サーベイランスは SSI 制御のための重要な手法であるが、低所得環境での最善のサーベイランス方法については明らかにされていない。
目的
ケニアの病院 1 施設において、SSI サーベイランスプログラムのいくつかの項目の疫学的特性を評価すること。
方法
手術創分類(SWC)スコアおよび米国麻酔学会(ASA)スコアの観察者間一致度を κ 統計量を用いて評価した。退院後の電話調査による SSI 検出を、「ゴールドスタンダード」である医師による外来患者の診察と比較した。米国疾病対策センター・全米医療安全ネットワーク(CDC-NHNS)リスク指標の諸項目によるSSI検出の的中率を、2010 年 8 月から 2011 年 2 月に産科婦人科の大手術を受けた患者を対象として評価した。
結果
適切な研修を実施後、外科医が判定した SWC スコアおよび麻酔科医が判定した ASA スコアのいずれについても、高い観察者間一致度が認められた。電話調査による退院後 SSI の検出は、感度 70%(95%信頼区間[CI] 47% ~ 87%)、特異度 100%(95%CI 95% ~ 100%)であった。産科婦人科の大手術を受けた 954 例の患者においては、CDC-NHNS リスク指標モデルの中で SSI リスクに関連するパラメータとしては、SWC スコアが唯一のものであった(オッズ比 4.00、95%CI 1.21 ~ 13.2、P = 0.02)。
結論
低所得環境の病院で SSI サーベイランスは実施可能であるが、そのためには熱心な病院職員、十分な研修、およびサーベイランス方法の部分的な修正が必要である。電話調査による退院後 SSI サーベイランスは不完全ではあるが、各診療所での検出方法としては実務的な代替手段となる。本研究では、SWC スコアが産科婦人科手術後の SSI リスクの唯一の予測因子であった。
