「広義の」ナッジを展開する:スコーピングレビューからの洞察★

『ナッジ』は病院内の感染対策の改善に重要な役割を果たす可能性がある。しかし、その枠組みは目新しいが、それらの目的、戦略および実行アプローチは新しいものではない。本レビューでは、病院内の 感染対策 において医療従事者を対象とした『ナッジ』介入に通常用いられる方法の概要を示す。最初の PubMed 検索ではヒット数が 9 件であった。そこで、検索基準を広げ、同じ目的、戦略およびアプローチを維持しながら、従来の『ナッジ』の枠組みを超えた情報源からの洞察を含む「広義の『ナッジ』」を取り入れ、2 回目の検索を行った。2 回目の検索では、PubMed、Epistemonikos、Web of Science および PsycInfo において、PRISMA ガイドラインに従って検索を行った。アブストラクトをスクリーニングし、学際的チームのレビュアーが、選択された論文の全文を読んだ。全体で 5,706 件の重複しない一次研究が特定された。このうち 67 件をレビューの対象としたが、狭義の『ナッジ』とされたものは 4件のみで、医療従事者の手指衛生の変化に焦点を当てた研究であった。すべての論文で、肯定的な介入結果が報告されていた。手指衛生遵守の向上に焦点を当てた論文 56 報のうち、71.4%で肯定的結果が報告された。医療機器の消毒については、50%の研究で有意な結果が示された。ガイドライン遵守への介入については、有意な結果の割合は 66.7%であった。『ナッジ』の枠組みで認められる戦略、方法、実行アプローチを採用した介入を含む、広義の『ナッジ』の概念が導入された。本研究の結果は、この概念によって、よりインパクトのある『ナッジ』の科学的展開が促進される可能性があることを示している。これは、臨床医や研究者、政策立案者が効果的な『ナッジ』介入を策定および実行する際に有用であると考えられる。
監訳者コメント:
『ナッジ』とは行動経済学の原理に基づき強制的な手法でなく人の行動変容を促す手法で、様々な分野での研究や応用が進んでおり、感染管理や抗菌薬適正使用においても注目されている領域である。本文中の Table II には『ナッジ』を利用した感染管理の論文の要点がカテゴリー別に整理され、これから『ナッジ』を利用した取り組みを検討している人は是非読んでおくと良いだろう。
