香港の心臓胸部ユニットのアウトブレイクにおけるカンジダ・オーリス(Candida auris)の広範囲にわたる空気拡散★

背景
多剤耐性真菌であるカンジダ・オーリス(Candida auris)による院内アウトブレイクは、世界的に報告が増加しつつある。その伝播様式は、通常は報告によれば直接接触を介するものである。これまでに一部の研究から、空気の大きな流れを引き起こす活動時の短距離の空気拡散が原因となっている可能性が示唆されているが、広範囲の空気拡散に関するエビデンスは少ない。
目的
地域病院 2 施設の 2 病棟(H7、5E)で発生した C. auris による院内アウトブレイクについて記述すること。
方法
直接接触および空気拡散が原因となる可能性を調べるため、サンプルを患者、病棟の環境表面(接触頻度の高い物品および手の届かない表面)から採取し、また受動的空気サンプル採取のために平板培地を設置した。今回のアウトブレイクで回収された C. auris 分離株について、疫学および系統樹解析も実施した。
結果
患者 18 例で、無症状の C. auris 皮膚保菌を有することが確認された。予想通りに、接触頻度の高い病棟の物品で採取された複数のサンプルから C. auris が分離された一方、2.4 m の高さにあって患者には手の届かない天井の換気用の格子で採取されたサンプル 2 個からも分離された。さらに、C. auris コホート区域から 9.8 m 離れた廊下のエアリターンの格子で採取されたサンプル 1 個も陽性であった。受動的空気サンプル 2 個(4 日間にわたり症例が確認されなかった病室 1 室で採取された 1 個)も陽性で、C. auris の空気拡散の可能性が示唆された。全ゲノムシークエンシングにより、空気、環境、および患者からの分離株にクローン性が認められた。
結論
本研究は、開放病室形式の病棟における C. auris の広範囲にわたる空気拡散の可能性を実証した初の研究である。C. auris アウトブレイク管理において、換気対策および手の届かない高い位置にある環境表面の汚染除去を検討すべきである。
監訳者コメント:
過去の報告ではベッドメイキングのような乱流を生じる活動によって、空気中に C. aurisが検出されていた。本事例では、C. auris の空気拡散は気流によって運ばれた C. auris を含む皮膚上皮との関連が示唆された。C. auris の空気拡散を防止するためには、接触予防策や環境表面の消毒だけでなく、換気対策も検討すべきである。
