Norovirus outbreaks in hospitals in China: a systematic review

背景

病院内のノロウイルスアウトブレイクは患者ケアを損ない、多大な金銭的損失をもたらす恐れがある。現時点で、中国本土における病院内のノロウイルスアウトブレイクに関する情報は限られている。

目的

アウトブレイクの速やかな特定と制御を促進するために、公表文献を系統的にレビューし、中国本土の病院におけるノロウイルスアウトブレイクの特徴を説明した。

方法

Preferred Reporting Items for Systematic Review and Meta-Analysis（PRISMA）基準に従ってシステマティックレビューを行った。PubMed、Web of Science および Chinese Journals Onlineデータベース（China National Knowledge Infrastructure［CNKI］、Chinese Wan Fang デジタルデータベース［WANFANG］）などのデータベースにおいて、発端から 2022 年 7 月 18 日までの検索を行った。

結果

2022 年 7月 18 日までに発生した中国の病院におけるノロウイルスアウトブレイク計 41件が、32 報の論文で報告されていた。報告されたアウトブレイクの大半は上海と北京におけるもので、12 月と 1 月に発生していた。症例は主に成人であった。男女比は 1.22：1 であった。ノロウイルスアウトブレイクにおける症例の大半が入院患者であり（56.82％）、15 件のアウトブレイクで医療従事者が罹患していた。ノロウイルスアウトブレイクは民間および公立病院、2 次および 3 次医療機関のいずれでも発生し、主に内科と老年科でみられた。ヒトからヒトへの伝播が主要な伝播様式であり、GII がより多く認められた。

結論

病院内のノロウイルスアウトブレイクは、患者と医療従事者のいずれにも影響を及ぼし、かなりの金銭的損失をもたらす場合がある。ノロウイルスアウトブレイクに起因する疾患負荷をより完全に理解するためには、病院内のサーベイランスを確立する必要がある。

監訳者コメント：

中国におけるノロウイルスアウトブレイクの事例が、どの程度正しく疫学的に導き出されているのか興味が深くある。中国の国内でも、正しい疫学評価はよく見極めないといけない。
