Processes of care in surgical patients who died with hospital-acquired infections in Australian hospitals

背景
感染症は、手術患者の入院に合併することがある。病院獲得感染症が、死亡した手術患者におけるケアのプロセスに及ぼす影響は明らかにされていない。
目的
病院獲得感染症が、院内で死亡した手術患者におけるケアのプロセスに及ぼす影響を検討すること。
方法
オーストラリアの全国手術死亡報告（2012 年から 2016 年）から外科医が記録した感染症のデータを抽出し、病院獲得感染症および感染症なしにグループ分けした。死亡報告には、院内で死亡したあらゆる年齢の手術患者が含まれていた。すべての患者が手術を受けていたわけではなかった。市中獲得感染症の患者および感染症発症時点に関するデータが欠けていた患者は除外した。多変量ロジスティック回帰を用いて補正を行い、病院獲得感染症がこれらの患者におけるケアのプロセスに及ぼした影響を明らかにした。病院獲得感染症に関連した費用を推定した。
結果
死亡した手術患者の 5 分の 1 は病院獲得感染症で死亡した（11,681 例中 2,242 例、19.2％）。病院獲得感染症患者では、感染症がなく死亡した患者と比べて、以下のケアのプロセスに増加が認められた：術後合併症（51.0％対 30.3％、補正オッズ比［aOR］2.20、95％信頼区間［CI］1.98 ～ 2.45、P ＜ 0.001）、予定外の再手術（22.6％対 10.9％、aOR 2.38、95％CI 2.09 ～ 2.71、P ＜ 0.001）および予定外の集中治療室入室（29.3％対 14.8％、aOR 2.18、95％CI 1.94 ～ 2.45、P ＜ 0.001）。病院獲得感染症患者は感染症のない患者と比べて、入院期間が長く、病院費用が多くかかった。
結論
病院獲得感染症は、死亡した手術患者において、ケアのプロセスおよび費用の増大と関連した。こうした転帰について、生存した手術患者において検討する必要がある。
