これは、COVID-19 パンデミック中の集中治療室（ICU）において、7 例の患者と 1 名のスタッフが罹患したPanton–Valentine ロイコシジン産生メチシリン耐性黄色ブドウ球菌（Staphylococcus aureus）（PVL-MRSA）のアウトブレイクについての報告である。10 か月の期間に亘って ICU で 6 例の患者が、PVL-MRSA の同じ株に感染し、1 例の歴史的な症例が ICU の外で特定された。この微生物が定着していた医療従事者と全症例がつながっていた。全身投与の抗生物質療法なしの局所の除菌療法の失敗は、伝播が続く結果となり、予防可能であった PVL-MRSA の感染を 1 件引き起こした。このアウトブレイクは、アウトブレイクに巻き込まれた医療従事者が必要とするかもしれない支援を特定している。また、このアウトブレイクに関係していたが分散していた症例や歴史的な症例を特定する際に全ゲノムシークエンシングが果たした役割も明らかにしている。これは、今度は、アウトブレイクの管理と医療従事者の支援における意思決定を助ける。この報告は、医療における PVL-MRSA に関連したアウトブレイクについての文献の再検討も含み、そのようなアウトブレイクにおける医療従事者の除菌レジメンや職場復帰の推奨の管理についての現在の国のガイダンスは見直しが必要であることをはっきりと示している。
監訳者コメント：
PVL 陽性 MRSA の 3 年間にわたるアウトブレイクにおける全ゲノムシークエンシングの活用事例。スタッフ不足の影響や創処置の問題、サーベイランスの必要性などについても言及されている。
