医療におけるアシネトバクター・バウマニー(Acinetobacter baumannii)の乾燥表面バイオフィルムの伝播に関する自動化接触モデル★

背景
乾燥表面バイオフィルムは、病院内の環境表面および機器表面に広く認められており、長期にわたり微生物汚染が残存しうる理由を説明し、院内感染の伝播重要な役割を果たす可能性がある。臨床環境において乾燥表面バイオフィルムがどのように形成され、増殖するのかについてはほとんど知られていないが、消毒の有効性を試験するための乾燥表面バイオフィルムモデルが存在する。
目的
本研究において、患者と表面の相互作用を模倣することで、病院内における乾燥表面バイオフィルムの形成を表現する新規のバイオフィルムモデルを開発した。
方法
本モデルは、人工ヒト汗と臨床的に重要な病原体の、ヒトの接触を模倣できる人工的に合成した親指を用いた伝播により、乾燥表面バイオフィルムを生成する。モデルバイオフィルムにおける DNA、複合糖質および蛋白質の組成、ならびに微小コロニーの構造的特徴を、落射蛍光顕微鏡により可視化した蛍光染色を用いて明らかにし、公表されている臨床データと比較した。
結果
顕微鏡法により、表面上におけるバイオフィルムの不均一性が示され、蛋白質が基質内の主要な構成要素であり、次いで複合糖質および DNA が多かった。本モデルは、微量の微生物および人工ヒト汗を、5 分ごとに最長 120 時間にわたり、ステンレススチール製金属片上に反復して送達して、平均 1.16 × 103 cfu/cm2 のバイオフィルムモデルを生成し、これは臨床的乾燥表面バイオフィルムで報告されている範囲内(4.20 × 102 ~ 1.60 × 107 個/cm2)であった。
結論
我々の in vitro 乾燥表面バイオフィルムモデルは、in situ で報告されている表現型上の多くの特徴および形質を示した。本モデルは、見逃されていることの多い重要な特徴を有しており、より現実的な微生物播種試験を用いて抗バイオフィルム活性があると主張するためなど、下流の応用が可能であることを示している。

本研究により乾燥表面バイオフィルムの新しい実験モデルが開発され、医療環境で問題となる乾燥表面バイオフィルムへの理解が深まることが期待される。
