手術室における隔離方策の遵守を改善する：シミュレーションに基づくトレーニングとビデオに基づくトレーニングの有効性および費用を比較する前向きシミュレーション研究★

背景
病原体の伝播経路によって異なる隔離方策が必要とされる。有効性および／または衛生方策に対する認識の改善について、異なる形態の衛生トレーニングを比較した研究はほとんどない。本研究は、手術室における異なる形態の隔離トレーニングのベネフィットについて、またそれぞれの形態が衛生方策の認識に及ぼす影響について、シミュレーションに基づくトレーニングとビデオに基づくトレーニングの比較によって評価することを目的とした。
方法
本多施設共同前向き無作為化対照試験では、施設内でのシミュレーションに基づくトレーニングと通常のビデオに基づくトレーニングを受けた後の医療従事者において、衛生に関する知識、心理的安全性および認識について比較した。
結果
どちらの形態のトレーニングも、知識または認識された心理的安全性における有意な改善をもたらさなかった（F ＝ 0.235、P ＝ 0.629、η2 ＝ 0.003）。シミュレーションに基づくトレーニングを受けた参加者は、設定されたシナリオにおいて意見を述べる意思のレベルが、ビデオに基づくトレーニングを受けた参加者よりも高いと報告した。参加者は、シミュレーションに基づくトレーニングを、ビデオに基づくトレーニングよりも有意により肯定的な認識をしていた。
結論
参加者の既存の知識レベルに基づいてトレーニングの目標を明確に定義することが、極めて重要である。今後の研究にとって、異なる形態のトレーニング後の衛生規制の実施に関する長期効果および継続的なベネフィットについて検討することは興味深いであろう。

参加者の知識レベルがすでに高い場合は、シミュレーションに基づくトレーニングが適している。
