背景
医療施設の製氷機は、アウトブレイクや偽性アウトブレイクが疑われてきた。また、アウトブレイクや偽性アウトブレイクに結び付けられたことさえあった。これらの機器の保守のガイドラインは存在するが、感染防御の明解で独立した標準はなく、微生物汚染についてもほとんど知られていない。
目的
医療施設の氷水機の中の微生物汚染、増幅、日和見病原菌の存在を評価すること。
方法
一般的な微生物指標(従属栄養細菌数、総細胞および無傷細胞)、糞便指標(腸球菌)、日和見病原菌(緑膿菌[Pseudomonas aeruginosa]、非結核性抗酸菌(NTM)、カンジダ[Candida]属菌)の濃度を、772 床の病院の患者病棟にあった 36 個の氷水機で測定した。微生物の増殖の場所を特定するために、5 個の氷水機および隣接する蛇口で、profile sampling を実施した。
結果
Candida属菌が氷水標本の半数で見つかり、腸球菌と緑膿菌が、それぞれ、6 個と11 個の排水管口に存在していた。NTMは、すべての氷水標本と、36 個のバイオフィルムのうち 35 個で測定された。前置フィルターと製氷機も増幅の場所で、5 分間フラッシュした製氷機の水の中のNTMの密度は、フラッシュした隣接する水道水と比べ、平均で、1.3 log10 高かった。
結論
製氷機のデザインを、微生物の増殖が減少するように適合させる必要がある。従属栄養細菌密度(現行の微生物指標)と NTM 濃度の間に相関がなかったことは、日和見病原菌とそれより良く相関する洗浄効率の指標が必要であることを示唆している。医療施設の製氷機の洗浄と消毒のガイドラインは改善する必要がある。特に、氷が最も病気に罹りやすい患者に与えられるときには。そして、NTM は効率の指標となる可能性がある。
監訳者コメント:
製氷機の衛生管理は不十分である。それは多くの感染管理担当者が内心思っていることであろう。本論文をきっかけに、一度自院の製氷機の管理を見直すと良いだろう。
