β-ラクタムアレルギーの評価に対する医療提供者の視点の定性分析

背景
β-ラクタムアレルギーを評価する際に医療提供者から報告される障害に関する理解は不足しているが、実用的かつ効果的な実施介入のためには、これらの障害を知ることが必要である。
方法
2021 年 9 月から 2023年 7 月の間に、集中治療、救急医療、感染症および総合診療の分野に従事する医師、看護師、薬剤師から成る医療提供者 25 名の面接を行った。これらの提供者のうち 23 名が米国で従事していた。理論的領域フレームワークに基づく半構造化面接ガイドを用いて、面接を行った。面接の筆記記録の演繹的および帰納的分析を行い、Behaviour Change Wheel を用いて介入の推奨事項に変換した。
結果
広く支持されている考えとして、アレルギーの評価に関する明確な方針がない、アレルギー情報の記録がわかりにくいまたは欠けている、情報が入手可能なときにアレルギーを評価する自身および同僚の能力を信頼している、β-ラクタムアレルギーを評価する能力が最も備わっている提供者は薬剤師である、などがあった。
結論
医療制度にβ-ラクタムアレルギーの評価に関する方針を採用して広め、可能であれば薬剤アレルギーの評価への薬剤師の任用を促進すべきである。電子カルテのアレルギーの欄を、アレルギー反応の曖昧でない記録を促し、以前に忍容されたβ-ラクタム系抗菌薬の使用を支持するように、修正すべきである。
監訳者コメント:
医療従事者の薬物アレルギーは一定の割合で存在し、各医療従事者はそのリスクを知っておく必要がある。抗菌薬は入院患者において高頻度に使用されることから、そのリスクマネージメントはしっかり行う必要がある。
