Appropriateness of antibiotic use for patients with asymptomatic bacteriuria or urinary tract infection with positive urine culture: a retrospective observational multi-centre study in Korea

背景

抗菌薬耐性は世界中で公衆衛生を脅かしており、抗菌薬の不適切な使用が主な原因の 1 つである。

目的

無症候性細菌尿（ABU）および尿路感染症（UTI）における抗菌薬の質的な使用を評価すること。

方法

2021 年 4 月に入院患者、外来患者および救急部門で実施された尿培養が陽性（≧10⁵ コロニー形成単位/mL）であった症例に関し、韓国の病院 26 施設でスクリーニングを行った。症例は ABU、下部 UTI、上部 UTI に分類された。ABU と UTI の診療ガイドラインに基づく質の指標を用いて、感染症専門家が抗菌薬使用の適切性を後向きに評価した。

結果

本研究は ABU または UTI の患者計 2,697 例を対象とした。ABU 患者 1,157 例、下部 UTI 患者 677 例、上部 UTI 患者 863 例において、抗菌薬使用の適切性を評価した。ABU 患者 1,157 例のうち、251 例（22％）は適切な適応のない抗菌薬を処方されていた。適切な適応の抗菌薬が処方されていた ABU 患者 66 例のうち、投与期間が適切であったのは 23 例（34.8％）のみであった。経験的な抗菌薬投与と確定的な抗菌薬投与の適切性が認められたのは、下部 UTI 患者ではそれぞれ 527 例（77.8％）と 353 例（68.0％）、上部 UTI 患者では 745 例（86.3％）と 583 例（78.2％）であった。抗菌薬の投与期間が適切であったのは、下部 UTI 患者では 321 例（61.8％）、上部 UTI 患者では 576 例（78.7％）であった。

結論

ABU と UTI における抗菌薬使用に関する今回の全国的な質的評価によって、抗菌薬が不適切に処方されることは多く、抗菌薬の投与期間は不必要に延長されていることが明らかになった。

韓国における尿培養陽性例における抗菌薬使用の実態が理解できる報告である。日本においても同様の研究を行ってみるのもよいかもしれない。
