背景

カルバペネム耐性グラム陰性桿菌が出現するのは、主に、無差別に長期にカルバペネムを使用することで促進される。このような使用は重要な寄与因子の 1 つである。

目的

カルバペネムの処方とカルバペネム耐性グラム陰性桿菌の臨床分離率の両方に対する2 つのカルバペネム抗菌薬適正使用支援プログラムによる介入の影響を、分割時系列分析を使って評価すること。

方法

韓国の 1 つの 3 次病院における 2017 年 1 月から 2022 年 7 月までのカルバペネムの使用状況に関するデータを使って、時系列分析を実施した。2 つのカルバペネム抗菌薬適正使用支援プログラムによる介入を連続して実施した。すなわち、(i)2018 年 11 月から 2020 年 4 月までの早期モニタリングとフィードバック(介入 1)と、(ii)2020 年 5 月から 2020 年 8 月までの事前承認(介入 2)であった。カルバペネムの毎月の使用状況、および各介入前と後のカルバペネム耐性グラム陰性桿菌の出現率を、自己回帰和分移動平均モデルを使って比較した。

結果

早期モニタリングとフィードバックを実施した結果、カルバペネムの使用量が有意に減少し、それに続く事前承認の実施後更に減少した。カルバペネム耐性大腸菌(Escherichia coli)および肺炎桿菌(Klebsiella pneumoniae)の出現率は、介入 1 の後に増加したが、介入 2 の後、増加傾向から変動しない傾向へと有意に変化した。カルバペネム耐性緑膿菌(Pseudomonas aeruginosa)の出現率は、ベースラインの期間に増加したが、介入 1 の後、変動がなくなった。カルバペネム耐性アシネトバクター・バウマニ(Acinetobacter baumannii)の出現率は、介入 1 および 2 を実施中、有意な減少が観察された。

結論

抗菌薬耐性細菌によって引き起こされる感染を防ぐためには、包括的な抗菌薬管理と厳格な感染制御を採用することが重要であることを、この研究は、はっきりと示している。

韓国の 3 次病院におけるカルバペネム系抗菌薬の適正使用支援活動の影響を検討した。カルバペネム系抗菌薬の早期モニタリングとフィードバック、事前承認によりカルバペネム系薬の使用量が減少し、カルバペネム耐性グラム陰性桿菌の検出状況も変化した。カルバペネム耐性アシネトバクターの検出率は減少したが、腸内細菌目細菌では有意な減少を認めなかった。抗菌薬適正使用支援活動と感染対策活動を推進していく必要がある。
