術前評価外来における薬剤師主導によるペニシリンアレルギー表示削除プロジェクト：容易に達成できる目標がすぐ近くにある★

背景
ペニシリンアレルギーの誤った表示は、入院期間の延長、クロストリジオイデス・ディフィシル（Clostridioides difficile）およびメチシリン耐性黄色ブドウ球菌（Staphylococcus aureus）感染のリスク増大と関連する。
目的
アレルギー専門医以外による使用のために策定されたペニシリンアレルギー表示削除ツールを評価すること。
方法
大規模の英国教育病院の術前評価外来を受診し、ペニシリンアレルギーを報告した患者は、必要に応じて表示が看護師または薬局職員により直接削除された。アレルギー専門医以外による使用のために策定されたペニシリンアレルギー表示削除ツールが適応、使用され、看護職員は誤った表示の削除ができるように支援情報および教育を提供された。抗菌薬専門薬剤師は、追跡調査、投与された予防的抗菌薬の照合、臨床記録の調査、および術後の患者への電話連絡を行い、発生したあらゆる副作用の詳細を得た。
結果
ペニシリンアレルギーを報告した患者計 163 例が介入の対象として特定された。ペニシリンアレルギー歴を報告し、表示の直接削除が適切とされた 29 例（17.8％）のうち 8 例（27.6％）は削除への同意を拒否した。それ以外の 21 例（12.8％）では表示が直接削除され、このうち 12 例（7.4％）は術前評価外来の予約中に同意し、残りの 9 例（5.5％）では抗菌薬専門薬剤師によって術後に同意が得られ、表示が削除された。
結論
術前評価外来は、誤ったペニシリンアレルギー表示を術前に直接削除できる、患者パスウェイにおける適切な場所および時点として確認された。この取り組みは看護職員により可能であったが、抗菌薬専門薬剤師による支援が相当数の患者における表示の削除を可能にしたことが、結果から示唆される。
監訳者コメント：
患者がアレルギーだと申告している事例の多くは、実際にはアレルギーではない、ということはよく知られているが、その問題に具体的に何か取り組んだ、という話はあまり聞かない。本論文は研究というより実践報告に近いが、参考になる内容だと思われる。
