What techniques are recommended to undertake procedures that require asepsis? Content and cluster analysis of information supplied in international guidance

背景

医療従事者は無菌状態を要する処置を頻繁に行うが、どのように無菌操作を行うべきかに関するエビデンスに基づいた明確なガイダンスは存在しない。

目的

国内および国際的ガイダンスにおける無菌操作の実施に関連する情報を比較し対比させるため、内容・クラスター分析を行うこと。

方法

内容・階層的クラスター分析。

結果

感染予防ガイドラインを作成し、他の論題に加えて感染予防の助言をし、処置（例えば創傷治癒）のガイダンスを作成する機関、実践マニュアル、MeSH および Wikipedia から、16 の情報源を特定した。内容は、無菌操作を支持する理論、使用された専門用語、いつどのように実施すべきかおよび器具に関連していた。情報の性質および量はばらつきが大きかった。最も高頻度に提示された情報は環境または器具（N ＝ 13）に関連しており、その後、無菌の絶対的な性質および手指衛生の重要性（N ＝ 10）、一般的な個人防護具、病原体の重要性および非接触操作（N ＝ 8）、リスクベースであること（N ＝ 7）、重要な部分または場所の存在、異なる種類の無菌操作があること（N ＝ 6）が続いた。最も包括的な情報源は、米国の創傷治癒機関および国際的に使用される英国の教科書であった。一部の一般的な感染予防ガイドラインでは、提供される情報はわずかであった。

結論

無菌操作に関連する研究および実践の向上は、共通の目標と理解の欠如によって難航している。本研究は、無菌操作を構成するもの、いつどのように実施すべきかおよび必要な器具に関して確立するための一歩である。本研究は実践、方針および教育を支援するために必要であり、医療資源の持続可能性を向上させる可能性がある。
