Applicability and limitations of quality indicator-based assessment of appropriateness in antimicrobial use: a comparison with expert opinion

背景

抗菌薬適正使用支援を効果的に実施するためには、抗菌薬処方の妥当性を事前に評価する必要がある。

目的

抗菌薬処方の妥当性の判断における品質指標(QI)の有効性を、専門家の見解の有効性と比較して評価した。

方法

本研究では、韓国の病院 20 施設において抗菌薬使用を調べ、感染症専門家が QI と専門家の見解に基づいてその妥当性を評価した。選択した QI は、入院患者については(1)血液培養を 2 回実施、(2)感染が疑われる部位の培養を実施、(3)ガイドラインに従って経験的抗菌薬療法を処方、(4)経験的治療から病原菌に対する標的治療に変更、また外来患者については上記(2)、(3)、(4)であった。適用性、QI の遵守度、QI と専門家の見解の一致度を検討した。

結果

全体で研究実施病院における抗菌薬の治療的使用 7,999 例を検討した。専門家は 20.5%(1,636/7,999 例)を不適切な使用と評価した。入院患者では、抗菌薬使用が QI の 4項目すべてに基づいて評価された割合は 28.8%(1,798/6,234 例)であった。外来患者では、抗菌薬使用例のうち QI の 3項目すべてを用いて評価されたのは 7.5%(102/1,351 例)のみであった。入院患者における専門家の見解と QI 全 4項目との一致度はわずかであり(κ = 0.332)、外来患者における専門家の見解と QI 全 3項目との一致度は弱かった(κ = 0.598)。

結論

QI は、抗菌薬使用の妥当性の判断において限界があり、専門家の見解との一致度が低かった。したがって、抗菌薬使用の妥当性を判断する際には、これらの QI の限界を考慮に入れるべきである。

抗菌薬適正使用のために、感染臓器の特定と起炎菌の同定・感受性検査は必須であり、初期の適切な経験的治療開始と検査結果に基づく標的治療への変更が患者予後の改善と耐性菌出現防止につながる。過去の研究において適正使用の指標 QI として 11 項目が選択されている。本研究ではそのなかの 4 または 3 項目を使用して、抗菌薬使用の適切性が評価されているが、個々の症例毎に適応が異なることがあり、なかでも薬物血中濃度や静脈投与から経口投与への変更などは一律に QI で抗菌薬の適切性を評価することは難しく、専門家の判断が必要となるため、まだまだ QI のみで抗菌薬の適正使用を評価することは難しい。
