Hand hygiene compliance and associated factors among healthcare workers in selected tertiary-care hospitals in Bangladesh

背景

手指衛生は患者安全の基本要素の 1 つである。医療従事者における手指衛生遵守は、リスクの認識、組織の文化、監査の方法、手指衛生用品の入手しやすさを含む様々な因子によって大きく異なっている。

目的

本研究は、バングラデシュの選択された 3 次ケア病院の医療従事者における手指衛生遵守および関連因子を評価することを目的とした。

方法

医師、看護師、清掃スタッフにおける手指衛生遵守を記録するため、2020 年 9 月から 2021 年 5 月の間に、3 次ケア病院 10 施設で WHO の「手指衛生 5 つのタイミングツール」を用いて非参加型観察を行った。手指衛生遵守に対する重要な障壁を明らかにするため、半構造化面接も実施した。

結果

14,668 回の手指衛生の機会を観察した。全体の手指衛生遵守率は 25.3％であり、看護師において最も高く（28.5％）、清掃スタッフにおいて最も低かった（9.9％）。公立病院の医療従事者は、私立病院の医療従事者よりも手指衛生実践の遵守のオッズが有意に高かった（補正オッズ比 1.73、95％信頼区間［CI］1.55 ～ 1.93）。患者と接触後の手指衛生実施のオッズは、患者接触前と比較して 3.36 倍高かった（95％CI 2.90 ～ 3.90）。手指衛生実施に対する重要な障壁は、供給品の不足（57.9％）、皮膚反応（26.3％）、作業負荷（26.3％）、設備の不足（22.7％）と報告された。全体として、医師は病棟の 81.7％で、看護師は病棟の 95.1％で観察された手指衛生用品を利用可能であったが、清掃スタッフ用に割り当てられた手指衛生の設備は確認できなかった。

結論

医療従事者における手指衛生遵守は、安全な患者ケアの基準にはるかに達しなかった。手指衛生用品の不十分な供給は、優先順位付け、感染予防・制御の促進および投資が欠けていることを示している。
