院内給水設備におけるレジオネラ・ニューモフィラ(Legionella pneumophila)を検出するための定量的リアルタイム PCR の検証★

背景
レジオネラ・ニューモフィラ(Legionella pneumophila)の迅速なモニタリングは医療施設におけるレジオネラ症のリスクを減少させるために不可欠である。しかし、培養結果に 8 日以上の時間を要し、是正措置の実行を遅らせている。本稿では定量的 PCR 法の性能を評価し、院内給湯設備における L. pneumophila を検出するための定量的 PCR の措置レベルの閾値を決定した。
方法
院内給湯設備から熱水サンプル(N = 459)を採取した。iQ-Check® Quanti リアルタイム PCR 定量キット(iQ-Check® Quanti real-time PCR Quantification kits、Bio-Rad 社)を用いて L. pneumophila を定量し、結果を培養と比較した。培養の措置レベルの閾値である 10 cfu/L、1,000 cfu/L に対応する定量的 PCR の閾値は訓練用データセットで決定し、独立データセットで検証した。
結果
培養および定量的 PCR で測定した L. pneumophila の濃度は、訓練用データセット(Spearman の相関係数 ρ = 0.687、P < 0.0001)と検証用データセット(ρ = 0.661、P < 0.0001)の両方と相関していた。培養の措置レベルの閾値 10 cfu/L に対応する L. pneumophila の定量的 PCR の陽性閾値は 91 ゲノムユニット(gu)/L(感度 86.4%、陰性適中度[NPV]93.3%)であり、培養の措置レベルの閾値 1,000 cfu/L に対応する陽性閾値は 1,048 gu/L(感度 100%、NPV 100%)であった。
結論
定量的 PCR による L. pneumophila の検出は、是正措置の実行を迅速化するためのモニタリング戦略における培養の補完として、院内で自信を持って実行できる可能性がある。

L. pneumophila の定量的 PCR は高い感度・陰性的中率を示したことから、迅速なスクリーニングに有用である。
