背景

重症急性呼吸器症候群コロナウイルス‐2(SARS-CoV-2)の病原性はパンデミック期間に変化した。本研究は、呼吸器感染症の治療の優先順位および院内感染制御策の適応の根拠を提示するために、野生株、アルファ株、デルタ株、オミクロン株 BA.1/2、オミクロン株 BA.5 の流行波の期間にインフルエンザウイルスまたは SARS-CoV-2 に感染した入院患者を対象に、集中治療室(ICU)での治療、人工換気の必要性、体外式膜型人工肺(ECMO)の必要性、および死亡について評価した。

デザイン

単施設後向き症例対照研究。

設定

ドイツの 3 次病院。

参加者

SARS-CoV-2 に感染した成人入院患者 1,316 例およびインフルエンザウイルスに感染した成人入院患者 218 例。

方法

患者の人口統計学的データ・転帰パラメーター・基礎となる併存疾患について、病院情報システムより入手した。リスク因子と転帰変数の有意な関連を評価するために多変量回帰分析を実施した。

結果

SARS-CoV-2 感染入院患者は、インフルエンザウイルス感染入院患者と比較して、野生株、アルファ株、デルタ株の流行波では、院内死亡率、ICU 入室、人工換気の必要性の割合が有意に高く、野生株流行波では ECMO の必要性の割合が有意に高かった。一方、オミクロン株 BA.1/BA.2、オミクロン株 BA.5 の流行波では、SARS-CoV-2 感染患者は、インフルエンザウイルス感染患者と比較して、院内死亡率、ICU 入室、人工換気の必要性、ECMO の必要性のいずれのリスクも有意に高くなかった。SARS-CoV-2 感染患者の入院期間は 10.8 日から 6.2 日に短縮し、これはインフルエンザウイルス感染患者の入院期間(8.3 日)よりも短かった。

結論

SARS-CoV-2 感染症とインフルエンザウイルス感染症において、治療容量を同等に割り当てるべきである。少なくとも感染症の重症度については、同レベルの感染制御策を適用できるであろう。

SARS-CoV-2 は変異ごとに病原性や伝播力、感染してからの重症度など多岐にわたり変化を遂げている。流行がなかなか収束しない厄介なウイルスである。
