Trends of antimicrobial use through selected antimicrobial indicators in Spanish hospitals, 2012 to 2021

背景

抗菌薬使用のサーベイランスは、抗菌薬耐性に取り組む管理戦略の重要な要素である。

目的

欧州疾病予防管理センターが提唱する 6 項目の指標を用いて、抗菌薬使用を評価した。

方法

2012 年から 2021 年の期間にわたるスペインの病院での抗菌薬使用に関する点有病率調査データを解析した。年別の各指標の記述的解析を、全体および病院の規模別に実施した。ロジスティック回帰モデルを用いて、有意な経時動向を特定した。

結果

全体で、患者 515,414 例と抗菌薬使用 318,125 例を解析の対象とした。抗菌薬使用率は、研究期間を通して安定していた（45.7％、95％信頼区間［CI］45.6 ～ 45.8）。全身投与された抗菌薬の割合および非経口的に投与された抗菌薬の割合に、わずかであるが有意な増加傾向が認められた（それぞれオッズ比［OR］1.02、95％CI 1.01 ～ 1.02 および OR 1.03、95％CI 1.02 ～ 1.03）。内科的予防投与のために処方された抗菌薬の割合および使用理由が患者のカルテに記録された抗菌薬の割合に、わずかな改善が認められた（それぞれ－0.6％および 4.2％）。外科的予防投与が 24 時間を超えて処方された割合は、2012 年の 49.9％（95％CI 48.6 ～ 51.3）から 2021 年には 37.1％（95％CI 35.7 ～ 38.5）に低下し、有意な改善が認められる。

結論

過去 10 年間のスペインの病院における抗菌薬使用率は安定していたが、高かった。解析を行った指標の大半には、ほとんどあるいはまったく改善が認められなかったが、例外として 24 時間を超える外科的予防投与の処方は減少していた。

2012 年から 2021 年まで 10 年間のスペインの約 400 病院の抗菌薬の使用状況に関する点有病率調査（PPS）。指標の多くは大きな変化がなかったということだが、例えば抗菌薬開始理由に関するカルテ記載率はもともと 80％を超えており、最初から改善の余地が少ないものもある。日本ではなかなか PPS が進まないが、引き続きの取り組みが期待される。
