Antimicrobial stewardship programme implementation in the UAE: perspectives of key stakeholders using Consolidated Framework for Implementation Research

背景

多くの研究は抗菌薬適正使用支援プログラム（ASP）の実施を報告しているが、これらの研究には理論的な基盤が欠けているという限界がある。このために、ASP 実施の成功または不成功に影響を及ぼし得る重要な因子が見落とされている可能性がある。

目的

アラブ首長国連邦の病院における ASP の実施に関する主要な利害関係者の視点を、促進因子と阻害因子に焦点を当てて探索すること。

方法

本研究は、個々の患者レベルでの抗菌薬の臨床使用に関与する ASP の利害関係者（ASP チームのメンバーと非メンバーを含む）との半構造化面接を用いた定性的方法を使用した。面接のスケジュールは発表済み文献に基づき、実装研究のための統合フレームワーク（CFIR）を基盤として作成、レビューおよび進行を行った。登録は、意図的なスノーボールサンプリングにより行った。面接は録音して文字起しを行い、2 名の独立した研究者が CFIR をコード化の枠組みとして用いて主題分析を行った。

結果

面接 31 件でデータの飽和が達成された。ASP 実施における促進因子または阻害因子として、CFIR の複数の構成概念が同定された。促進因子には、外部方針の要請（国内的および国際的の両方）、リーダーシップによる支援、利害関係者の関与、協力を勧める文化、効果的なコミュニケーション、ならびに先を見越した計画立案などが含まれた。阻害因子には、責める文化、ASP 実施の複雑さ、ならびに専門スタッフの不足などが含まれた。

結論

本研究において、利害関係者の視点から、ASP 実施に対する多くの促進因子と阻害因子が同定された。必要なリソースの提供を支援するための早期のリーダーシップによる関与の重要性、効果的な計画立案と複数の関与技術の確立の必要性、ならびに医療提供者との有用なコミュニケーションが、診療の改善を支援するものとして特定された主要な推奨事項である。

本研究により、ASP 実施に関する促進因子と阻害因子が判明した。各国や地域の実状を考慮した上で、ASP を実施する必要がある。
