Nurses’ knowledge and implementation of antimicrobial stewardship and infection prevention strategies in acute paediatric settings

背景

看護師は患者と最初に接する医療従事者であり、感染症の徴候についてモニタリングおよび報告を行う責任を負っている。COVID-19 パンデミックは、感染予防において看護師がリーダーシップをとる役割を担うことを強化した。それにもかかわらず、抗菌薬スチュワードシップの取り組みにおける看護師の貢献については依然として十分に認識されていない。

目的

3 つの急性期小児科病棟において、小児科看護師が抗菌薬スチュワードシップおよび感染予防制御の実践における自身の役割と貢献についてどのように理解しているかを明らかにすること。

方法

都市部にある 3 次小児科病院の思春期病棟、腫瘍科病棟および外科病棟から看護師 43 名を登録し、探索的な質的記述研究を実施した。

結果

主題および内容分析により 3 つの主題、すなわち予防可能な感染症についての理解、患者を守るためのエビデンスに基づく包括的なガイドライン、ならびに感染症の予防制御および抗菌薬スチュワードシップにおける役割がデータから導き出された。関連する副主題は、COVID-19 に対して鈍感になってきていること、感染予防・制御に関する予防措置の理解、病院の方針およびガイドラインの正確な実施、電子カルテの使用に関連する制限、敗血症の管理および適時の微生物検査の重要性に関する理解、抗菌薬スチュワードシップにおける役割について曖昧であること、利用者（訳者注：患者およびその保護者）教育に高い優先度を置くことであった。

結論

看護師の自身の役割に対する理解は、手指衛生を実施すること、標準予防策、およびリスクの高い抗菌薬の使用について報告することなどの実践に焦点が当てられていた。小児における COVID-19 伝播と症状に関する理解の欠如も報告された。感染予防および抗菌薬スチュワードシップのベストプラクティスに関する教育は、看護師と保護者の双方にとって極めて重要であると認識されていた。

オーストラリアの単施設の 3 つの小児病棟において、抗菌薬スチュワードシップおよび感染予防戦略の実践に対して看護師の役割と貢献の理解を明らかにした研究。世界中の医療機関で抗菌薬スチュワードシッププログラムの実施が推奨されており、すべての臨床医の積極的な参加を促進するための方針が定められているが、看護師の貢献については十分に認識されていない。看護師の抗菌薬スチュワードシッププログラムの関与を考える上で参考になる。
