Influence of bed making on loads of airborne and surface-associated drug-resistant bacteria in patient rooms

緒言

患者のベッドメイキング後の空中多剤耐性菌の動態に関する入手可能な情報は限られている。メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus ［MRSA］）の負荷に及ぼすベッドメイキングの影響に関する以前の試験を、大きなサンプルサイズで再評価し、多剤耐性グラム陰性菌の環境負荷について同時試験を初めて実施した。

方法

MRSA 患者の病室 26 室、多剤耐性グラム陰性菌患者の病室 25 室において、ベッドメイキング前後（1 分前および 1 分、15 分、60 分後）に、ベッドから 0 m および 3 m 離れた位置で空中病原体を計数し、患者周囲の表面サンプリングを実施した。影響を及ぼす可能性のある因子を記録した。

結果

非病原性グラム陽性菌種が空中サンプルの大部分を占める一方で、グラム陰性菌はわずか 1.4％であった。ベッドメイキングにより、コアグラーゼ陰性ブドウ球菌（staphylococci）および黄色ブドウ球菌へと割合が転じた。ベッドメイキング 1 分後および 15 分後の大部分のサンプルで、室内空気中の MRSA の一過性増加が認められた。2 つの病室の空気中で多剤耐性グラム陰性菌が検出された。患者環境における表面サンプルより、通常は、多剤耐性グラム陰性菌ではなく、MRSA が分離されることが示された。ベッドメイキング後の空中病原体と表面病原体の負荷の間に相関が確認された。

結論

多剤耐性菌保有患者のベッドメイキングをする場合、慎重な処理方法と併せてフェイスマスクを着用することの重要性が研究結果から示された。患者の保菌状況が不明な場合は、ベッドメイキング中およびベッドメイキング直後に居合わせている職員や他の患者への防御策に配慮すべきである。ベッドメイキング後少なくとも 30 分間は、表面消毒を開始すべきではない。

監訳者コメント：

ベッドメイキングの後に空中の MRSA やグラム陰性桿菌が一過性に増えることを示した論文。ちなみに真菌は調査していない。ベッドメイキングの際に、空中の微生物に変化が生じていることは容易に予想されることではあるが…また見てはいけない課題を一つ見つけてしまった気持ちである。
