日本の歯科大学病院における薬剤師主導による抗菌薬適正使用支援プログラムの多面的な介入★★

背景
歯科医療において抗菌薬が処方されることは多い。しかし、この領域では抗菌薬の不適切な処方がよくみられる。歯科医療における抗菌薬処方の最適化には、処方行動の改善を図る効果的な対策が必要となる。
目的
経口抗菌薬処方数 100 件あたりの各薬剤群での毎月の抗菌薬処方率について、薬剤師主導による多面的介入の有効性を評価すること。
方法
大学病院歯科外来において前後比較試験を実施した。薬剤師主導の多面的介入には、処方に対する即時および直接のフィードバック、ペニシリンアレルギー患者に対する抗菌薬処方基準の説明、歯科学生の指導、病院が認可した抗菌薬の再検討を含め、2017 年 4 月から 2022 年 3 月まで実施した。抗菌薬は以下の 8 群に分類した。ペニシリン、第 1 世代および第 2 世代セファロスポリン系、第 3 世代セファロスポリン系、フルオロキノロン系、マクロライド系、クリンダマイシン、カルバペネム系、その他である。
結果
試験期間中の歯科外来受診は計 2,643,988 件であった。介入前から介入期間にペニシリンの月平均処方率は、経口抗菌薬処方 100 件あたり 45.6 件から 84.1 件に増加した（P ＜ 0.001）。その一方で第 3 世代セファロスポリン系では 43. 0 件から 7.3 件に減少した（P ＜ 0.001）。さらに、フルオロキノロン系、マクロライド系、カルバペネム系の月平均処方率は、100 件あたり11.2 件から 7.44 件に減少した（P ＜ 0.001）。
結論
介入により、歯科医の抗菌薬処方行動が改善され、その結果、ペニシリンの毎月の処方件数が直ちに増加し、第 3 世代セファロスポリン系、その他の広域スペクトル経口抗菌薬の毎月の処方件数が同時に減少した。
監訳者コメント：
東京医科歯科大学ならではの、薬剤師主導の歯科医療における経口抗菌薬の適正使用に関する取り組み。この領域における抗菌薬適正使用の取り組みには大きなポテンシャルがある一方で、国内の成果が JHI のようなジャーナルに紹介されることは少なかった。本研究は世界への発信はもちろん重要だが、日本国内でも広く紹介されることが期待される。
