患者の付き添い人および面会者の手指衛生遵守：観察研究★

背景
医療関連感染症を引き起こす多くの微生物の拡散に対して、手指衛生は簡便かつ低コストの予防策である。
目的
本稿は、患者の付き添い人および面会者の手指衛生遵守を調査するためにデザインされた記述的研究である。
方法
トルコ西部の大学病院に入院した患者の付き添い人および面会者 209 人を本研究の対象とした。人口統計学的質問票、手指衛生質問票、手指衛生実践の観察フォームを用いて、データを入手した。
結果
患者の付添人および面会者のうち、96.2％は院内滞在中に手洗いの重要性に関して指導を受けなかったとし、74.6％は感染症の予防に手洗いがきわめて重要であるとした。患者家族の回答では、体液に触れた後は、ほとんどの場合、手指を洗っているが（91.7％）、患者に触れる前に手指を洗うことは稀であった（34.0％）。観察ではこれらの割合が低下し、患者に触れる前の手洗いの割合はもっとも低く（22.4％）、患者の体液による汚染リスクがある場合は、手洗いの割合がもっとも高かった（68.6％）。
結論
患者の付添人および面会者は、院内滞在中に手指衛生の重要性に関する指導を受けておらず、観察された手指衛生遵守率は、回答の場合よりも低かった。推奨事項として、患者の付添人や面会者に対して、さまざな指導方法による計画的な手洗い指導をすることと、より長期的な観察研究を実施することが挙げられる。
監訳者コメント：
これまた「どうなってるんだろう？」と疑問には思われているがあまり研究のなかった、患者の付き添いや面会者を対象とした手指衛生の遵守率に関する研究。96.2％が手指衛生については特に指導を受けていないが、74.6％が手指衛生が重要だと認識していると回答した一方で、直接観察では手指衛生の遵守率は自己申告より低かった。
病院にはまだまだ取り組まないといけないことがたくさんあります…。
