小児におけるカルバペネム耐性グラム陰性菌のその後の腸内除菌と関連する修正可能なリスク因子:前向きコホート研究★

背景
カルバペネム耐性グラム陰性菌に起因する院内感染症は、死亡率の増加および長期入院と関連する。したがって、カルバペネム耐性グラム陰性菌のその後の除菌は、臨床上および公衆衛生上、重要な意義を有する。
目的
小児におけるカルバペネム耐性グラム陰性菌に対するその後の腸内除菌の修正可能/修正不可能なリスク因子を検討すること。
方法
3 次病院に入院した(2018 年から 2019 年)カルバペネム耐性グラム陰性菌保菌者(生後 1 日から 16 歳)を対象とした。カルバペネム耐性グラム陰性菌保菌の検出にあたり、患者が入院している場合は、直腸スワブ培養物を週 1 回採取し、退院後は月 1 回、12 か月間採取した。カルバペネム耐性グラム陰性菌の除菌の定義は、1 週間以上の間隔で採取した直腸スワブ培養物が 3 回連続して陰性であることとした。修正可能(投薬/医療機器)および修正不可能(年齢/性/併存疾患)なリスク因子を記録した。カルバペネム耐性グラム陰性菌のその後の除菌に対して Cox 回帰分析を実施した。
結果
カルバペネム耐性グラム陰性菌保菌者 130 例が登録された。5.4%の患者が 12 か月後も保菌状態にあった。その後の除菌のリスク因子は、免疫抑制(ハザード比[HR]0.52、95%信頼区間[CI]0.31 ~ 0.87)・カルバペネム(HR 0.52、95%CI 0.30 ~ 0.91)・プロトンポンプ阻害薬(PPI)(HR 0.39、95%CI 0.24 ~ 0.64)とこれらの使用期間、入院期間(10 日あたりのHR 0.90、95%CI 0.81 ~ 0.92)、再入院の回数(HR 0.90、95%CI 0.86 ~ 0.96)、腹部手術(HR 0.33、95%CI 0.17 ~ 0.65)、尿路カテーテル(HR 0.42、95%CI 0.24 ~ 0.76)、ステロイド投与期間(10 日あたりの HR 0.86、95%CI 0.84 ~ 0.88)であった。
結論
小児において、カルバペネム・PPI とこれらの使用期間、ステロイド使用期間、免疫抑制、尿路カテーテル、再入院、入院期間、および腹部手術が、カルバペネム耐性グラム陰性菌のその後の除菌と関連する。その後の除菌のリスクがある小児患者には、選択的スクリーニングおよび事前の接触予防策を考慮すべきである。カルバペネム耐性グラム陰性菌のその後の除菌のリスクがある既知の保菌者には、より長期にわたる接触予防策を慎重に適用していく必要がある。
監訳者コメント:
「多剤耐性のグラム陰性桿菌ってどのくらいの期間保菌が続くのでしょうか?」というよくある質問に対する小児の研究。12 か月後でも 5.4%が保菌していた。また保菌持続のリスク因子としては、免疫不全、カルバペネム系薬の使用、プロトンポンプ阻害薬などといった、これまたよく知られたリスク因子が挙げられた。
