Incidence, complications, and costs of peripheral venous catheter-related bacteraemia: a retrospective, single-centre study

背景

末梢静脈カテーテル（PVC）合併症は平均で患者の約半数に発生し、PVC の中途抜去、継続中の治療法の中止およびカテーテル交換を必要とする。

目的

PVC に起因する血流感染症（BSI）の現在の発生率、合併症および費用を推定すること。

方法

PVC 関連 BSI を有する患者（症例群）と PVC 関連 BSI を有さない患者（対照群）をマッチさせた。

結果

2018 年 1 月 1 日から 2020 年 3 月 31 日までに、救急部を受診した患者 113,068 例中計 9,833 例（9％）が PVC 挿入後に内科病棟に入院した。そのうち 581 例（6％）が血液培養で 1 回以上陽性を示した。これらのうち 25 例（4％）が PVC 関連 BSI を有すると判断された。主要な合併症は患者 9 例に認められた。患者 1 例は集中治療室に 11 日間の入室を要する重症敗血症を呈し、その後に長期の抗菌薬療法を要する胸椎（T4 ～ T7）脊椎椎間板炎を呈した。他の患者は同様に長期の抗菌薬療法を要する僧帽弁心内膜炎を発症した。患者 1 例は初回の PVC 感染の 3 か月後に仙骨前膿瘍を発症し、ドレナージのため 19 日間の再入院を要した。入院費用の中央値（四分位範囲）は症例群で 11,597（8,479 ～ 23,759）ユーロ、対照群で 6,789（4,019 ～ 10,764）ユーロであり、追加費用の中央値は 5,587 ユーロとなった。

結論

内科病棟に入院した患者における PVC 関連 BSI の発症リスクは低いと思われるかもしれないが、PVC 関連 BSI の合併症は重症であり、関連死亡率は依然として高い。これらの合併症の治療に用いられる財源は、高品質の材料・技術の使用および医療提供者の改善されたトレーニングを含む予防に費やすほうが良いであろう。

監訳者コメント：

末梢静脈カテーテルは日本においても高頻度で使用されているが、決して感染に対する適切なリスク管理が実施されているわけではない。挿入から抜去までの間の無菌的操作が重要であるが、この無菌的バリアを突破して BSI が発生する。クリーンベンチを使用した無菌的な輸液調製や側管からの注射時の無菌的操作も BSI のリスクを軽減する上で重要となる。
