フランスの病院における基質特異性拡張型β-ラクタマーゼ(ESBL)産生腸内細菌目細菌の院内伝播の評価を目的とした定量的アンチバイオグラム法の使用★

背景
基質特異性拡張型β-ラクタマーゼ(ESBL)産生腸内細菌目細菌は、病院内で高い検出率が認められるが、従来のタイピング法による院内獲得のリアルタイムでのモニタリングは難しい。さらに、患者間伝播には主な細菌種、すなわち大腸菌(Escherichia coli)と肺炎桿菌(Klebsiella pneumoniae)の間で違いがあり、同じ予防策を適用することの妥当性が疑問視される。
目的
定量的アンチバイオグラム(QA)法を疫学データと組み合わせて用いること(併用 QA 法)で ESBL 産生腸内細菌目細菌の交差伝播イベントを検出すること、ならびに ESBL 産生腸内細菌目細菌に対する標準予防策または接触予防策の有効性を判定すること。
方法
最初に、バリデーションセットを用いて、併用 QA 法の妥当性を、パルスフィールド・ゲル電気泳動(PFGE)と疫学データの併用(併用 PFGE 法)と比較した。次に、入院患者において ESBL 産生腸内細菌目細菌の交差伝播イベントを検出するため、4 年間にわたる後向き解析を実施した。2 種の細菌、すなわち ESBL 産生大腸菌および ESBL 産生肺炎桿菌についてスクリーニングを行った。本研究の実施期間中、ESBL 産生大腸菌が検出された患者に対しては標準予防策のみを行ったが、ESBL 産生肺炎桿菌が検出された患者には接触予防策を行った。
結果
概念検証として、交差伝播イベントの検出を目的とした併用 QA 法 および併用 PFGE 法の使用により、大腸菌については同じ結果が、肺炎桿菌については少なくとも 75%に対して同様の結果が得られた。全体で、ESBL 産生大腸菌が分離された患者 722 例、および ESBL 産生肺炎桿菌が分離された患者 280 例を、本後向き解析の対象とした。大腸菌および肺炎桿菌について、それぞれ 9 件および 23 件の交差伝播イベントが特定され、それぞれ20 例(2.7%)および 36 例(12.8%)の患者が含まれた。
結論
併用 QA 法は、交差伝播イベントを検出を目的とした疫学的サーベイランスを行う上で迅速なツールとなる。本研究を行った病院では、ESBL 産生大腸菌獲得の予防には標準予防策で十分であったが、ESBL 産生肺炎桿菌獲得の予防には接触予防策を実施しなければならない。
