個々の手指衛生の改善と医療関連感染に対する効果：電子手指衛生モニタリングシステムを用いた長期追跡調査研究★

背景
人それぞれの手指衛生行動に関する詳細な知見を得ることは、最も効果的な長期手指衛生遵守介入を策定するために重要な役割を担う可能性がある。
目的
電子手指衛生モニタリングシステム（EHHMS）により提供される2種類のフィードバック介入が、持続的な手指衛生遵守の改善、個別の応答性、院内血流感染（HABSI）および院内尿路感染（HAUTI）の予防に及ぼす効果を検討する。
方法
本研究は、介護者 142 名と医師 39 名が従事する内科において 4 年間観察された、2 つの 2 年コホート（EHHMS に曝露された群と曝露されていない群）を対象とした。医療従事者をベースラインの成績に基づき4群に層別化し、予測される介入への応答性を評価した。
結果
すべての医療従事者において、ベースライン時の成績とは無関係に手指衛生遵守は上昇したが、例外として低成績群の数名は常に手指衛生遵守が低かった。ベースライン時の成績が低かった 2 群では、グループフィードバックへの応答性（手指衛生遵守の週間変化 4.4％および 3.1％）が、個別フィードバックへの応答性（手指衛生遵守の週間変化 1.0％および 2.2％）と比較して高かった。HABSI 症例数は介入期間中に有意に減少し（P ＝ 0.01）、黄色ブドウ球菌（Staphylococcus aureus）に対する効果が最も大きかった。HAUTI には有意な変化は認められなかった。
結論
EHHMS 介入により手指衛生遵守の改善が順調に持続し、HABSI 症例数が減少した。ほぼすべての医療従事者が介入に応答した。ベースライン時の成績が低かった 2 群は、高成績群と同程度の手指衛生遵守に達することはなく、さらなる改善の余地と、個別化された介入を強化する必要性が示された。
監訳者コメント：
同じ条件で定量的な手指衛生活動の評価を行えば、その結果に基づく医療従事者の手指衛生行動の成績が明らかとなる。
