背景

新型コロナウイルス感染症(COVID-19)パンデミックの経過において、医療施設でのアウトブレイクが重要な役割を果たした。

目的

2020 年 3 月から 2022 年 5 月までのドイツの院内、外来診療、およびリハビリテーション施設における重症急性呼吸器症候群コロナウイルス 2(SARS-CoV-2)アウトブレイクについて検討すること。

方法

ドイツの義務的届出システムのデータを用いて、年齢および性別ごとにみた症例数、症例致死率(CFR)、およびアウトブレイク症例について記述した。Pearson 相関を用いて、ワクチン接種キャンペーンの開始前と開始後について、一般集団における症例の動態を、医療関連感染症(HAI)としての SARS-CoV-2 アウトブレイクにおける症例と比較した。さらに、反事実的シナリオを用い、ワクチン接種以前の期間をベースラインとして、予防された HAI 症例数を推定した。

結果

2022 年 5 月末までに、医療関連アウトブレイク 8,941 件および症例 73,626 例が認められた。症例のうち 51,504 例は院内、15,524 例は外来診療、6,598 例はリハビリテーション施設であった。アウトブレイク 1 件当たりの症例数中央値は 4 例(範囲 2 ~ 342)であり、症例は女性で多く報告され、46,818 例(63.6%)であった。全 CFR は 8.1%で、男性(12.4%)が女性(5.7%)より高かった。ワクチン接種キャンペーンが完全に実施された後、一般集団における発生率上昇とその後のアウトブレイク症例との関連の強さは 10 分の 1 に低下した。さらに、反事実的分析から、2021 年末までに、55,000 件を超えるアウトブレイク症例が予防できた可能性が示唆された。

結論

ワクチン接種キャンペーンを非薬理学的方策と組み合わせることが、医療関連アウトブレイクの症例数、規模、および CFR を低減するための鍵となった。

監訳者コメント

ドイツでの届出システムを活用した新型コロナウイルス感染症の医療関連アウトブレイクの解析。予防接種キャンペーン導入後に医療関連の集団発生の数、規模、致死率が大幅に減少していることがわかる。一方で、オミクロン株流行期であるフェーズ 4 ではフェーズ 3 よりも医療関連感染によるアウトブレイクが 2 倍になっており、オミクロン株流行期においてはワクチンの医療関連感染の抑止効果が低下している。
