日本のオミクロン株およびデルタ株流行期の医療従事者における SARS-CoV-2 感染のリスク因子の比較★★

背景
医療従事者における新型コロナウイルス感染症(COVID-19)のリスク因子は、免疫逃避性の高いオミクロン株の登場以降に変化した可能性がある。
目的
デルタ株流行期とオミクロン株流行期の医療従事者における重症急性呼吸器症候群コロナウイルス 2(SARS-CoV-2)感染のリスク因子を比較すること。
方法
東京の医療研究センターのスタッフに対して反復して行われた血清調査のデータを用いて、2 つのコホートとして、デルタ株流行期コホート(N = 858)およびオミクロン株流行期コホート(N = 652)を確立した。潜在的なリスク因子について、質問票を用いて評価した。急性/現在または過去の SARS-CoV-2 感染を、PCR 検査または抗ヌクレオカプシド抗体検査によりそれぞれ同定した。Poisson 回帰を用いて、感染リスクのリスク比(RR)を算出した。
結果
SARS-CoV-2 感染のリスクは、オミクロン株流行期初期のほうがデルタ株流行期よりも 3.4 倍高かった。COVID-19 関連病棟で勤務していることも、SARS-CoV-2 への職業曝露の程度が高いことも、両流行期における感染リスク上昇には関連していなかった。オミクロン株流行期には、密閉空間、密集空間、および密接状況でマスクの装着なしに 30 分以上過ごしたことが多いスタッフでは感染リスクが高かった(3 回以上 対 なしの比較で RR 6.62、95%信頼区間 3.01 ~ 14.58)。一方、デルタ株流行期にはそのような関連は認められなかった。
結論
COVID-19 関連作業への職業曝露は、デルタ株流行期にもオミクロン株流行期にも SARS-CoV-2 感染のリスクと関連していなかったが、オミクロン株流行期には高リスクの行動は感染リスクの上昇と関連していた。

日本のオミクロン株およびデルタ株流行期の医療従事者における新型コロナウイルス感染のリスク因子を比較した報告。血清学的な調査が行われており、無症候ないしは軽度の症状で未診断となっているものも含めて感染を補足できている点が本研究の強みである。オミクロン株流行期ではデルタ株流行期より医療従事者の感染リスクは増加しており、リスクの高い行動と感染防止習慣の不十分な遵守が感染リスクの増加に関連していた。ナショナルセンターである国立国際医療研究センターからの報告であり、一読をおすすめする。
