背景
医療においてクロストリジオイデス・ディフィシル(Clostridioides difficile)感染症(CDI)と関連する主要な因子を病院組織レベルで特定すること。
デザイン
2011 年から 2019 年にわたる縦断的研究。病院報告を分析して、CDI の件数と病院に関連する複数の環境因子、すなわち経済的リソース(例、清掃費支出)、空間的リソース(例、患者用浴室付き個室の数)、人的リソース(例、医師スタッフおよび看護スタッフの人数)、ならびに文化的リソース(例、エラー報告の文化)について明らかにした。環境因子と CDI との関連を、病院内および病院間の複合ランダム効果モデルにおいて解析した。
設定
英国国民保健サービス(NHS)において稼働している一般病院トラスト計 129 施設。
参加者
2011 年から 2019 年にわたる NHS の一般病院トラスト 129 施設における全入院患者。CDI 症例 120,629 例を含む。
主要転帰評価項目
病院トラスト 1 施設当たりの年間 CDI 件数。
結果
個室は、病院内レベルでは CDI 件数が少ないことと関連したが、病院間レベルでは関連しなかった。同様に、看護スタッフが多いことは、病院内レベルでは CDI 件数が少ないことと関連したが、病院間レベルでは関連しなかった。この効果は、医師スタッフでは認められなかった。文化的リソースの保護効果については別の状況が明らかになり、病院間の差には弱いが有意な効果が認められたが、病院内の差にはそうした効果は認められなかった。経済的リソースは、病院間レベルでも病院内レベルでも縦断的にみて CDI との関連はなかった。
結論
本研究は、病院のリソースが CDI 発生率に有益な影響を及ぼすことを明らかにした。医療組織は、この知識を積極的な CDI 予防に利用することができる。
監訳者コメント:
病院のリソースが CDI 発生率の減少に寄与することを明らかにした論文である。医療関連感染対策において病院のリソースが重要であることを示している。
