目的
重症急性呼吸器症候群コロナウイルス-2(SARS-CoV-2)パンデミック時に、歴史的コホートおよび同時的コホートの患者を対象に院内感染(HAI)のリスク因子を評価すること。
デザイン
後向きコホート研究。
設定
米国ミズーリ州の病院 3 施設における 2017 年 1 月 1 日から 2020 年 9 月 30 日までのデータ。
参加者
患者 18 歳以上、入院期間 48 時間以上。
方法
HAI のリスク因子を明らかにするために、死亡を競合リスクとして扱う単変量・多変量 Cox 比例ハザードモデルを適用した。尿、血液、呼吸器の検体培養に基づく HAI の定義のロバスト性を評価するために、事前の感度分析を実施した。
結果
コホートにおいて入院件数は 254,792 件、HAI は 7,147 件(2.8%)(血液 1,661 件、尿 3,407 件、呼吸器 2,626 件)であった。SARS-CoV-2 患者は HAI のリスクが高く(補正ハザード比 1.65、95%信頼区間[CI]1.38 ~ 1.96)、SARS-CoV-2 感染は、HAI 発症の最大のリスク因子の一つであった。HAI の他のリスク因子は、特定の入院業務、慢性併存疾患、初回入院時の集中治療室への入室、超過体格指数、病院、選択的薬物投与などであった。HAI の低リスクと関連する因子は、入院した年(研究の過程を通して減少)、入院業務、薬物療法であった。肺炎/上気道感染症、尿路感染症の診断コードがある患者に限定した感度分析では、HAI のリスク因子は同様であった。
結論
SARS-CoV-2 は HAI リスクの有意な増加と関連した。
監訳者コメント:
この結果からも分かるとおりに、SARS-CoV-2 感染者は HAI のリスクが高い。元々 HAIのリスクの高い患者がこの理由に挙げられるだろう。
