病院における病原体伝播の代理としてラムダファージを使用するための方法論★

背景

入院患者における重大な懸念は、表面、患者、および医療従事者から媒介された病原菌による感染症の獲得である。医療関連感染症を制御するための基本となるのは、病原体の感染源の特定、その伝播を引き起こすプロセスのモニタリング、ならびにその伝播を制御するために用いられる手順の有効性の評価である。

目的

上記の目的を達成するために、バクテリオファージラムダ(λファージ)を用いた方法を提示すること。

方法

遺伝子標識を付けた複数のラムダファージを特定の濃度で、再現性の高いマネキン上で汚染のホットスポットとして知られる部位に播種した。次いで、事前に清掃された疑似病室に医療従事者が入室し、マネキンに対して一連の患者ケア業務を実行した。サンプル採取を、医療従事者の白衣および手指、ならびに事前に規定した病室における接触頻度の高い表面について実施した。サンプル採取後、効果的であることが示されている手順を用いて病室の除菌を行った。疑似試験の終了後、サンプルについて上記のラムダファージの存在、同一性、および濃度を検査した。

結果

生成されたデータにより、感染源の特定、ならびに確立された感染制御実践における医療従事者の不遵守が引き起こした汚染の規模の確認が可能となった。この技術により、単一の患者ケアエピソード中に、複数の汚染物質に対して標準化された追跡が可能となった。他の生物学的代理とは異なり、ラムダファージは病院で一般的に用いられている消毒薬に対して感受性があり、病原体伝播についてより正確な評価が可能である。

結論

この方法の適用は、医療関連感染およびその拡散において医療従事者の挙動が果たす役割に焦点を当てていたが、他の環境における微生物汚染の発生源および汚染箇所の特定に用いることが可能と考えられる。
